SummaryTheophyllin and aminophyllin are, as well known, powerfull pneumodilator drugs. Unfortunately, they are very irritant when used as aerosols in dyspneic patients ; furthermore, precipitation of the drugs takes places in the aerolizers.A salt of theophyllin, namely theophyllin-ethanoate of diethylene-diamine (Etaphylline), has been studied. This salt may be used in man as a 10 p. cent solution without discomfort nor irritation ; it does not cristallize when aerolized.In healthy subject, Etaphylline aerosols provoke a pneumodilation; they stop a dyspneic attack induced in normal man by aerosols of carbaminocholine.In dyspneic patients, Etaphylline aerosols may release bronchospasm and effect longduring improvement.

SummaryA case of Fanconi’s syndrom is described in an adult male, 56 years old. Physical examination showed deformities of the chest and back suggesting collapsed vertebræ. X rays were confirmative, showing true and pseudofractures in the pelvis and the hips. Clinical examination suggested nervous involvement with bilateral pyramidal signs.Laboratory studies of the blood showed : serum phosphorus±1,0 mgr%, serum calcium 9,2 mgr %, sugar and alcaline reserve are normal. Glucose tolerance test is normal despite the presence of sugar in the urine (8 to 10 gr/a day). Large amounts of P (0,5 gr/a day) are excreted despite the low level of blood phosphorus. Aminonitrogen in blood is normal (5,0 mgr %). Urinary excretion is high (0,55 gr/a day) when the patient is fasting. Paper chromatography of the urine showed an increased excretion of aminoacids when compared to that of normal people.Kidney function studies have shown, with normal filtration rate a decreased capacity to reabsorb glucose and phosphorus. The maximal capacity to reabsorb glucose was 181 mgr with a filtration rate at 100 cc/min. The maximal capacity to reabsorb phosphorus, when-the blood level was increased to 10 mgr%, was 1,5 mgr/min, a very low value when compared to normal values of 4,6 (±1,2) mgr/min. 86%of the filtred phosphorus appeared in the urine at plasma levels around 10 mgr %. At plasma levels as low ns 1,0 mgr %, 16 % were still excreted.Staticin caronamid did not change the renal excretion of phosphorus in this case, a fact that seems confirmative of the view that phosphorus in Fanconi’s syndrom is not excreted by tubular secretion.Administration of large amounts of tricalcic phosphate and of monosodic phosphate brought improvement to the patient. 6 months later osteomalacia was largely controlled. Neurological signs had disappeared. Serum phosphorus and calcium were only little changed, but alcaline phosphatases were increased from 5,8 C. R. to 12,1 U. R.The clinical state was much improved. The patient was able to move arms and legs, to sit on the edge of his bed without help. After five years confinement in bed or chair, walking seems a near possibility.

SummaryThe authors have observed a woman, 56 years old, with multiple adenomata of the parathyroid glands.The patient underwent parathyroidectomy at three times from deoember 1945 to July 1946, each of these explorations permitted to remove a parathyroid adenoma. Histological pictures of the three glands were very different : adenoma with big wasser-clear cells, tubular or alveolar formations, adenoma of the type said“endocrine adenoma”were found, sometimes in the same gland. Removal of the 3 adenomata corrected only partially the humoral picture ol the disease. Radiologically, no change was observed. It must ho pointed out that each of the surgical explorations did only show one adenomatous gland and it seems likely that the 2 adenoma removed afterwards developed after the first operation was performed.Excretion of P by the kidneys was used as a test of parathyroid activity in this case. The low phosphorus serum level was increased by intravenous administration of phosphates. Filtration rate was appreciated by the clearance of endogenous creatinine. The levels of excreted P were plotted against plasma levels and compared with those of 19 normal people studied by the same procedure. It was noted that the renal excretion of P in this patient was much higher than that of normal people. This was true not only at normal blood levels, but also when the blood concentration was increased enough to saturate the maximal capacity of the tubules to reabsorb phosphates.This observation does not agree with the conclusions of Jahan and Pitts who consider that parathyroid extracts do not change the maximal capacity of the renal tubules to reabsorb phosphorus.Removal of the first adenomatous gland was followed by a dicreased excretion of phosphates. The two last operations were only followed by a slight change in the renal function : a fact suggesting the persistanee of another adenomatous gland, perhaps in the neighbourghood of the thymus.
