SummaryThe effects of semi-synthetic human insulin (IHSS) and monocomponent porcine insulin (IPMC) were compared in a double blind study carried out in 9 totally insulin dependent diabetic patients in hospital. Short-acting preparations of these insulins were administered subcutaneously 4 times per day at an identical dose in each subject. Plasma glucose and plasma free insulin were measured 28 times on the 3rd day of each insulin treatment. At none of the 28 time points were the mean plasma glucose and free insulin levels significantly different. In the 5 patients without or with only a low titer of anti-insulin antibodies (AIA—), the mean circadian glycemia was markedly higher (under IHSS or IPMC) and the glycemic fluctuations were more pronounced (with IPMC) than in the 4 other subjects with a higher titer of anti-insulin antibodies (AIA +). This observation could be explained by a reduced half-life of circulating insulin in patients AIA—, leading to low levels of plasma free insulin, and, thus, to glycemic escapes, several hours after the insulin injection.In conclusion, the administration of identical doses of IHSS or IPMC to totally insulindependent diabetics (with or without anti-insulin antibodies) results in undistinguishable plasma glucose and free insulin circadian profiles. The presence of anti-insulin antibodies could improve the glycemic control by curtailing the fluctuations of plasma free insulin between insulin injections.

SummaryThe authors report the case of a 86-year-old patient with a Sezary’s syndrome. Clinically they emphasize the uncommon minimal dermatologic lesions and question the origin of the cutaneous lym phomas. The relation between these cutaneous lymphomas and the other T-cell malignancies is briefly discussed. The leukemic cells are in this case highly typical and their morphology and immunologic functions are described.No specific treatment has been applied.The patient died from a pneumopathy two months after his admission.

SummaryA 36-year-old woman was admitted to the hospital for surgical treatment of a fourth recurrence of right-sided spontaneous pneumothorax. During thoracotomy histologically proved endometriosis of the pleura overlying the right hemidiaphragm was found. About forty days before she had undergone a total hysterectomy and bilateral oophorectomy for pelvic endometriosis, followed by oestrogen implantation.In the discussion a review of the signs and symptoms of thoracic endometriosis is given, along with possible mechanisms of pathogenesis and different ways of treatment.

SummaryA sonographically guided fine needle aspiration biopsy of a well circumscribed echodense liver nodule disclosed a hepatic localization of a malignant plasma cell dyscrasia in a 51-year-old female. On computed tomography (CT) the lesion presented as a hypodense nodule. Further examinations revealed overt non-excretory multiple myeloma with intracellular kappa chains. Differential diagnosis of an echogenic hepatic mass should thus include extramedullary plasmocytoma.

SummaryA patient with acute lymphoblastic leukaemia developed bilateral pleural effusions without parenchymal lung lesions after administration of methotrexate. Repeated administration resulted in reappearance of the effusions. This syndrome may represent a less severe form of the known methotrexate associated pneumonitis. It may be due to a direct toxic effect of methotrexate or to drug hypersensitivity.

SummaryThe observation of a patient with Caroli’s disease, i.e., congenital dilatation of the intrahepatic bile ducts is reported. The profound abnormalities of the biliary tree, observed long before the initial description of the disease, were the source of the initial clinical picture which led to surgery. The surgical procedure which included a cholecystectomy together with a choledocoduodenostomy was followed by a number of episodes of relapsing cholangitis, two of whom required further surgical drainage of the common bile duct and intrahepatic biliary tree. The diagnostic approach and the surgical procedure (Roux-en-Y hepaticojejunostomy) performed twenty years later are described and discussed with reference to an updated review of the literature concerning this uncommon condition.

SummaryAcquired immunodeficiency syndrome (AIDS) has been described among homosexuals in the United States and recently in Europ.Two homosexuals from Belgium are reported with the syndrome. The first one was lymphopenic and died with cerebral toxoplasmosis; at autopsy no underlying cause of immunodeficiency was found.The second one had oral candidosis and chronic lymphadenopathy. Lymphocyte dysfunction was shown by a decrease of the helper/suppressor ratio (0.13) and a decrease of the response to mitogens.Electron microscopic studies showed tubuloreticular structures in the ganglion’s lymphocytes.AIDS may be found among Belgian homosexuals who should be considered like elsewhere as a high risk group.
