SummaryThis paper dcscribcs three renal allotransplants patients presenting hypertension shortly after transplantation. After exclusion of acute rejection by renal biopsy, an intravenous digital substraction angiography was performed showing a significant stenosis of the renal transplant artery-Percutaneous transluminal angiography was successful in two patients. One of them had an improvement of the renal function. In the third patient PTA was technically impossible; subsequent surgery was also not without problems.

SummarySarcoidosis was confirmed by biopsy in 13 out of 16 patients in which the disease was suspected. Hypercalcemia was present in 4 patients, associated with renal insufficiency; hypercalciuria without hypercalcemia was found in 3 patients with a normal glomerular filtration rate; 9 patients had normal serum and urinary calcium. Circulating 1, 25-dihydroxyvitamin D (l,25(OH), D) was increased in hypercalcemic patients, in spite of renal insufficiency. Corticosteroid treatment rapidly normalized l,25(OH)2D levels in a few days, and corrected hypercalcemia. In one patient, the progressive reduction of prednisolone to 2.5 mg/day was followed by the recurrence of hypercalcemia. One patient exhibited before corticotheraPy a recurrent seasonal hypercalcemia. The absence of calcium metabolism abnormalities in about 50% of our patients could correspond in some cases to inactive sarcoidosis and/or to a low level of 25-hydroxyvitamin D, the precursor of l,25(OH)2D, but remained without explanation in other patients. Calcium metabolism abnormalities thus do not appear as a rule in active sarcoidosis with a normal vitamin D supply. Hypercalcemia in sarcoidosis, related to l,25(OH)2D overproduction, seems to occur only in the presence of renal insufficiency. Acta Clin Belg. 42,6; 421–30.

SummaryFifty-six patients with severe intra-abdominal infection have been included in a controlled randomized open study. Imipenem/cilastatin was administered to 28 patients. The remaining 28 patients received ceftazidime, including 7 patients who received a combination of ceftazidime with metronidazole. The difference in cure rates did not reach statistical difference, although 3 failures were reported in the ceftazidime group and none in the imipenem/cilastatin group.Based on 100 pretreatment antibiograms, far more organisms were susceptible to imipencm (99) than to ceftazidime (80) (p<0.001). The Presence of a pathogen resistant to therapy in the follow-up culture was observed more frequently in the ceftazidime group (p<0.05).Tolerance and safety profile were good in both groups.It is concluded that imipenem/cilastatin is an efficient empiric monotherapy in severe intra-abdominal infections.

SummaryInfection with human parvovirus B19 has been documented in two persons with hereditary spherocytosis during a crisis of acute erythroblastopenia and also in a woman suffering from polyarthralgia, whose child developed erythema suggestive of fifth disease, a few days earlier. The etiologic role of HPV was suggested by the detection of HPV-particles (EM: 1/2), virus-specific antigens (CIE: 1/2) and viral DNA (dot-blot hybridization: 1/3) in serum samples of the patients as well as by the presence of significant anti-HPV IgM antibody levels (2/3). Apart from acute erythroblastopenia, which is now currently associated with HPV infection in persons with hereditary spherocytosis and other congenital hemolytic anemias, wc also recommend to look for infection with HPV in any case of polyarthropathy of recent onset.

SummaryA 28 year old woman with systemic lupus erythematosus presented with hepatomegaly and ascites.Clinical, histological and radiological investigations demonstrated the Budd-Chiari syndrome. The patient had two risk factors which could induce hepatic vein thrombosis: use of oral contraceptives and presence of the lupus anticoagulant. Despite fibrinolytic therapy followed by warfarin and corticosteroid, the patient died of hepatic failure five months after diagnosis. The so-called idiopathic Budd-Chiari syndrome could be due, at least in some patients, to the thrombogenous action of the lupus anticoagulant.

SummaryA questionnaire on smoking behaviour was sent to a randomized sample of Belgian Physicians in 1983. The replies showed that 32% were smokers, 33% ex-smokers and 35% non-smokers. Smoking was less prevalent among the younger physicians (P<0.05). Rather surprisingly 26% of the smokers presented a«consonant»behaviour, i.e. they declared that they did not want to stop smoking and that they had never tried to do so. Consonant smokers agreed, however, that smoking is harmful, and, therefore, have adopted less harmful smoking habits: they inhale less, smoke smaller amounts and smoke more cigars and pipes and less cigarettes than the 47% dissonant smokers i.e. those who declare that they would like to stop smoking and have already tried to do so.
