SummaryGas gangrene (GG) is an acute necrosis of the muscle and/or subcutaneous tissue duetogas-forming microorganisms. The authors retrospectively review 48 consecutive cases of clostridial GG treated in an intensive care unit over a 17-year period (1969–1986). Surgery and trauma were the most frequent initiating factors (88 %) of clostridial GG. The diagnosis is clinical and based on a comparison of the local and systemic signs. Direct examination of the sweet-smelling discharge, revealing large Gram-positive bacilli, confirmed the clinical impression in about 75 % of the cases. The key to successful treatment of clostridial GG requires the judicious application of resuscitation measures (86 % of our eases), a correct antibiotherapy (penicillin G, 30 to 40 million units daily), surgery (fasciotomies, excisions limited to undoubtedly dead tissue) and hyperbaric oxygen (H.B.O.) therapy (83% of our patients), in order to break the vicious circle of progression of the disease. Sixteen of our 48 patients with clostridial GG died in hospital.

SummaryA patient with heparin induced thrombocytopenia and subsequent disseminated intravascular coagulation and arterial thromboembolism is described. This rare complication of heparin therapy is often difficult to recognize. Pathophysiology is only partially understood. Heparin therapy must be stopped and another form of anticoagulation should be started. Surgery is sometimes necessary to treat the complications. When heparin induced thrombocytopenia is not recognized, it may lead to major complications and sometimes even to death. The diagnosis is confirmed by the demonstration that the serum of the patient causes platelet aggregation in the presence of heparin.

SummaryBetween February 1983 and December 1987, 262 invasive isolates ofHaemophilus influenzaewere sent to the national reference laboratory in Belgium. Type b strains were more commonly isolated from children (95 %) than from adults (55%) (p<0.0001). The prevalence of (lactamase production significantly increased from 18.5 % in 1983 to 35 % for the period 1986–1987 (p<0.05) and was somewhat higher among type b strains (29.5 %) than among other isolates (22.5 %). A strong correlation was found between resistance to ampicillin, chloramphenicol and tetracycline. Eleven B-lactamasc producing strains (17%) were resistant to chloramphenicol (p<0.001); all 13-lactamase negative strains were susceptible to chloramphenicol. Ampicillin resistant strains lacking 6-lactamase activity were not found. Monitoring the antimicrobial susceptibility ofH. influenzaeis an important component of a surveillance system forH. influenzaeinfections.

SummaryBacterial endotoxins have been studied for a long time. The discovery of the Limulus Amoebocyte Lysate test, offers a valuable tool to detcct endotoxins at very low concentrations in various body fluids.The usefulness of endotoxin detection in peritoneal fluid, urine, cerebrospinal fluid and plasma is reviewed. Anti-LPS immunotherapy is discussed.

SummaryTetracycline, doxycycline and minocycline are true broad spectrum antibiotics which remain the primary agents againstChlamydiae, Rickettsiae, Brucella, Pasteurella, Borrelia, Mycoplasma, Ureaplasmaand some additional specific agents of infectious diseases. The usefulness of these antibiotics against common gram-positive and gram-negative bacteria is dependent on the incidence of acquired resistance, which is mostly plasmid-mediated. Among gram-positive organisms tetracycline resistance is widespread in streptococci and coagulase-negative staphylococci. However, minocycline is generally active on the latter organisms. The activity against gram-negatives is unpredictable at present and susceptibility tests arc therefore mandatory. Some species, especiallyE. coliare more susceptible to minocycline than to the other compounds. Tetracyclines are frequently active on non-fer-mcntative bacilli other thanPseudomonas aeruginosa.They are primary agents for en-teropathogenic organisms, but here also, susceptibility tests are mandatory in order to exclude acquired resistance. The same holds true for many fastidious organisms such asHaemophilusand gonococci. From the other side, no acquired resistance has been demonstrated up to the present inBranhamella catarrhalisandChlamydiae.Although tetracyclines arc active on many anaerobic organisms, they arc no longer indicated against such bacteria.
