SummarySelective digestive decontamination (SDD) is a method that aims to eradicate the oropharyngeal and gastrointestinal carriage of potentially pathogenic micro-organisms (PPMO) by means of the application of high oropharyngeal and faecal antimicrobial concentrations. The SDD technique has proven its effectiveness in patients with profound granulocytopenia and has become commonplace in most European haematological services. A large number of trials most often uncontrolled or relying on historical controls have evaluated the use of SDD in intensive care patients.This article gives a summary of the effects of SDD on: colonisation of the different mucosae, infection, hospitalisation-time, mortality, cost-benefit aspects and the selection of resistant micro-organisms.

SummaryNeutrophilic eccrine hidradenitis (NEH) is a rare neutrophilic dermatosis occurring most frequently during induction chemotherapy for a variety of malignancies. We report a case of NEH in a 41-year-old woman with acute myeloblasts leukemia under daunorubicin, cytarabine and etoposide chemotherapy. She developed red, tender and painful nodules on a shoulder. The lesions resolved spontaneously. Histological examination is mandatory as the clinical presentation of this dermatosis is highly polymorphic. Leukemia cutis, sepsis, deep fungal infection and Sweet’s syndrome must be excluded as these implicate different therapies.

SummaryA case of double neutrophilic population in a more or less symptomless patient is described. The diagnosis is made on one hand by the presence of 2 distinct neutrophil clusters on the PEROX display of the H* 1 hematological automate, on the other hand by cytochemical peroxidase staining which clearly confirms the existence of two populations. One of the populations shows normal MPO activity, whereas the second one is characterized by partial MPO deficiency.

SummaryWe describe a case of meningitis due to Cryptococcus neoformans var gattii in an AIDS patient. The reasons for the observed predominance of C.neoformans var neoformans infections in AIDS patients living in areas endemic for the gattii variety are not understood thus far.Treatment with fluconazole resulted in poorclinical and mycological response. In vitro sensitivity testing of the fungus revealed high minimal inhibitory concentrations, suggesting fluconazole resistance. Treatment with amphotericin B was successful.
