SummaryThe value of laparotomy with splenectomy was proved in 52 of 76 patients laparotomized. This surgical approach resulted in a modification of the instituted treatment in all operated patients. The proposed surgical approach is recommended in patients with a haematosarcoma in whom an abdominal localization has already been demonstrated, in lymphoma patients who have undergone a course of treatment with radiotherapy or chemotherapy, in patients with disturbed tests indicative of the inflammatory function as well as in all Hodgkin patients in stage IV excluding the extra-abdominal localizations.

SummaryThe authors have studied 57 cases of Hodgkin’s disease in which laparotomy was performed (46 primary and 11 recurrent cases).They consider that laparotomy is indispensable for a more precise diagnosis of the disease and particularly of its extent and degree which is usually difficult to assess by clinical examination alone. They specify when it is indicated and suggest the contraindications.Finally, they are of the opinion that laparotomy is irreplaceable in designing an adequate treatment.In the framework of radiotherapy as the primary treatment of Hodgkin’s disease, the placing of metallic clips during the laparotomy reduces the volume which will be irradiated and an anatomo-pathological examination permits the immediate initiation of chemotherapy if there is an invasion of the liver or as required if stabilisation does not occur in cases of lympho-histio-plasmocytic infiltration of the liver.Laparotomy also has the advantage of allowing ovary pexis in a zone which is not irradiated. This is interesting both from a therapeutical as well as a psychological point of view.

SummaryTwenty-six untreated patients with Hodgkin’s disease were submitted to exploratory laparotomy and splenectomy. We found a significant inaccuracy of the pre-operative diagnostic measurements of intra-abdominal disease, except for the lymphography procedure.In ten cases (38%) the pre-operative assessment of staging required revision with a direct influence on management decisions in eight patients.

Summary36 lymphoma patients (CLL, lymphosarcoma, reticulosarcoma, Hodgkin’s disease) were investigated for immunochemical disorders. 7 patients had an M-component. 2 of them had 4 M-components simultaneously. 3 patients had AIHA. The data from the literature are discussed in the light of the newly developed fluorescence methods. The necessity of using new techniques is stressed in view of new classifications of lymphoproliferative diseases.

SummaryThirty patients with generalized Hodgkin’s disease were treated with a combination of Mustine, vinblastine and procarbazine. Prednisolone was added only for symptomatic reasons. After six courses a maintenance treatment with vinblastine alone was given for two years. In 16 out of 21 patients who had previously received no treatment or only radiotherapy, a complete remission was obtained (76%).Where monochemotherapy had already been given, the results of the combination treatment were clearly inferior. All patients with a complete remission have been free of signs of disease for periods ranging from 3 to 40 months. Although leukopenia is frequent, toxicity was not a major problem.

SummaryCombination chemotherapy with vincaleucoblastine, nitrogen mustard and methylhydrazine, is still able to induce 70 % complete and incomplete remissions in advanced stages of Hodgkin’s disease, most of them having already been treated previously. Patients with a Karnofsky index below 50 derive no benefit from this therapeutic scheme.

SummaryTen patients with malignant lymphocytic lymphoma (diffuse or nodular) have been studied in the leukaemic phase of their disease. The leukaemic cells of the blood and in some cases the bone marrow, a lymph node or a pleural effusion were labelled with tritiated thymidine in vitro and/or in vivo. This made it possible to show that in the peripheral blood, the fraction of cells in DNA synthesis (S) is greater than in the“true”myeloblasts and lymphoblastic leukaemias. The“S”fraction was also found to be variable from patient to patient and in any one patient from one site to another (blood, marrow, lymph node, pleural effusion).The labelling of these cells made it possible to study the mode of action of Vincristine on the cell cycle, twenty-four hours after an injection of 2 mg/m2. The % of lymphoma cells labelled and/or blocked in mitosis when compared to that of the erythroblasts in the same marrow sample, did not suggest a preferential effect on the malignant cells. Moreover, the % of cells labelled with tritiated thymidine in vitro was not compatible with a process of recruitment of lymphomatous cells in“S”by Vincristine.The effect of cytosine arabinoside was studied in one case. It was found to destroy a great number of lymphoma cells without decreasing the proliferative activity which took place at the same time in the surviving lymphoma cells.

SummaryThe authors administered a combination chemotherapy with nitrogen mustard, vincristine, procarbazine and prednisone to 37 cases of Hodgkin’s disease at advanced stages.Very encouraging results were obtained in previously untreated patients: 82.3% remissions with a plateau around 80 % remission and survival durations up to four years. Thus, the cure of such an advanced disease becomes conceivable, but the appearance of secondary cancers is possible.On the contrary, in previously treated patients, the results are less favourable: only 30% remissions are achieved with a median survival duration of responders around two years. Moreover, the haematologic toxicity is not negligible in this last group.

SummaryIn a series of 38 patients treated for advanced Hodgkin’s disease with the MOPP combination chemotherapy of DeVita, Serpick and Carbone, the complete remission rate was 84%. In this series 1/4 of the patients had been previously treated with other methods. Most of them, with a relapse of their disease, had a larger extension of their disease in comparison with the initial stage.In the evaluation of complete remission, the persistence of a splenomegaly (3 patients) is not necessarily a sign of incomplete remission; the histological study of the three spleens demonstrated no involvement of Hodgkin’s disease.The treatment is generally well tolerated. For the whole group, no more than an average decrease of 10 to 15% of the theoretical dose, mainly of the alkylating agents, was applied. The decrease was more important in patients who didn’t respond to the treatment; this allows us to suppose that haematological intolerance is a factor in bad prognosis.With a view to determining the best therapy for maintenance of complete remission, the patients were randomized, once complete remission had been obtained, into four groups, each receiving a different therapy.
