SummaryThe clinical and bacteriological observation of a 23-year old male student with gonococcal endocarditis is presented. In spite of antibiotic treatment and of four successive surgical interventions with insertion of artificial aortic and mitral valves, the patient expired as a consequence of haemodynamic problems.The pathogenesis of disseminated gonococcal in fection and the problems involved in bacteriological diagnosis are discussed.

SummaryThe differential diagnosis between multiple myeloma and benign monoclonal gammopathy has been investigated on the basis of a statistical model incorporating simultaneously four parameters, which refer to the cellular monoclonal event. The combination of the amount of bone marrow plasma cells, their monoclonal fraction and the concentration of the monoclonal immunoglobulin in the serum has been proven to give an optimal classification of patients with multiple myeloma and benign monoclonal gammopathy. Symptom free patients belonging to this group, are of special interest for the clinical follow-up, as they present an initial high monoclonal plasma cell mass.

SummaryAldosterone, discovered thirty years ago, has rapidly gained an enviable status among clinicians as well as biologists because of its key role as regulator of extra-cellular fluid volume—essentially through stimulation of sodium reabsorption in the distal parts of the nephron. This process has been fruitfully studied by taking advantage of amphibian epithelia carrying out transcellular sodium transport and resembling in this respect the distal renal tubule.Recent results suggest that, at least after prolonged incubation, not only is sodium entry (at the apical border) facilitated by aldosterone, but the latter steroid also somehow improves the handling of the ion by the«pump»located at the basal-lateral border. As a consequence, the target cells exposed to aldosterone are optimally geared for enhanced transcellular sodium transport.Biochemically, both of these aldosteronestimulated steps depend on what is nowadays considered the standard pattern of steroid hormone action: after interaction of aldosterone with protein receptors) present in the cytoplasm of target cells, and migration of the resulting complex to their nu cleus, synthesis of specific messenger ribonucleic acid is stimulated which leads to increased synthe sis of proteins eventually responsible for the physiological response.The biophysical and biochemical studies aimed at elucidating the mechanism of action of aldosterone continue to draw, heavily from therapeutic drugs such as spironolactone and amiloride. This illustrates anew how useful it is to link ever more tightly clinical Medicine and«basic»medical Sciences.

SummaryTen reports arc reviewed, presenting a total of 2 1 cases of acutc renal failure, ascribed to sulphinpyrazone therapy, usually early after myocardial infarction. A minority of these patients had a sulphinpyrazone induced allergic acute interstitial nephritis. Although acute uric acid precipitation may play a pathogenetic role, the arguments in favour of this mechanism are not convincing. It is suggested that in some cases an inhibition of vasodilatory renal hormones (prostaglandins and/or kallikrein kinins) could mediate renal functional im pairmcnt. Renal function should be closely monito red in patients taking this drug.
