SummaryThromboses are a major complication of several cardiovascular diseases. The following factors intervene in their formation. First, the haemodynamic factor can explain the differences in the genesis of venous and arterial thromboses. Second, the parietal factor depends on the subendothelial structures—collagen fibres, basal membranes, microfibrils—which can all fix the circulating platelets The latter are a third and essential factor in the genesis of thromboses. After their adhesion to the subendothelial structures, they undergo a mutual aggregation, leading to the formation of mural or occlusive thrombi.The understanding of the factors determining platelet adhesion and aggregation and the discovery of agents inhibiting these processes will allow an efficient treatment of incipient thromboses.

SummaryThe role of platelets in the thrombogenic process is reviewed in the light of recent experimental data. There is good evidence that intimal lesions are initiating a process which represents both haemostasisandthrombogenesis, by exposing subendothelial sites attractive to circulating platelets. In turn, activated and aggregated platelets may induce pathological modifications of the vascular wall, especially proliferation of smooth muscle cells. Finally activated (“nasty”) platelets embolize in the microcirculation, and might be responsible for metabolic, toxic and inflammatory lesions in the absence of ischaemia.

SummaryPlatelet aggregation plays an important role in the pathogenesis of vascular thrombosis, although the stimulus to platelet aggregation differs in arterial and venous thrombosis. In arteries, platelets aggregate following the interaction between platelets and exposed subendothelial structures, in a ruptured atherosclerotic plaque. In veins, aggregation follows the activation of the intrinsic mechanism of coagulation and the generation of thrombin in areas of retarded blood flow. In both cases, the platelet aggregate is rapidly stabilised by fibrin.

SummaryThe platelets from patients with thromboembolism or from patients at high risk for atherosclerosis, appear to have an increased adhesiveness and a high sensitivity to various aggregating agents. Although other methods of investigating the platelet function are available (e.g. clot retraction, thromboelastography, the Chandler loop, the electrophoretic migration of platelets) they do not have the same analytical value and are less commonly used. According to recent studies and our own experience, measurement of the adhesiveness and aggregation of platelets are more selective and reproducible techniques which can reveal decisive information in the diagnosis of haemorrhagic disorders. Making use of crossed experiments made feasible by recent methods of separation and resuspension of platelets, one can now distinguish intrinsic platelet abnormalities from functional disorders due to plasmatic abnormalities. These new methods may also reveal important information on the behaviour of platelets from patients with thromboembolism and will help to define the real significance of pharmacologic agents which inhibit the aggregation of platelets in vitro and in animals.

SummaryThe pharmacological properties of drugs inhibiting platelet function have been reviewed. Some of the problems arising in experimentation of potentially useful substances have also been discussed. Animal species, anticoagulants, plasma proteins and drug disposition are the main factors influencing platelet function tests. Clinically useful drugs inhibit platelet function by either increasing the levels of platelet cyclic AMP (PGE1, dipyridamole, pyrimido-pyrimidine compounds) or inhibiting the platelet biosynthesis of one or more intermediates appearing during the formation of PGE2and PGF2αfrom arachidonate (aspirin, indomethacin). Possibly the two mechanisms are interrelated in some way. Knowledge of the biochemical pathways underlying platelet function, should result in the development of more potent and more selective agents, potentially useful as antithrombotic drugs.

SummaryIn this paper the antiplatelet and antithrombotic activity of Dipyridamole is described. In in vitro tests the effect on platelet adhesion was found to be more pronounced than that on different types of aggregation.In in vivo models a good and long lasting inhibiting effect on primary haemostasis was seen in mice. An antithrombotic effect was achieved in a thrombus formation test in rats. The dose of Dipyridamole used in these in vivo experiments was only 10mg/kg orally.

SummaryA number of platelet function tests (retention tests, electrophoretic platelet mobility, platelet turnover, induced and spontaneous aggregation) have been used to confirm the role of enhanced thrombocyte adhesiveness or aggregation as risk factors for progressive atherosclerosis and its complications, with conflicting results.«Spontaneous»platelet aggregation using PAT I has been studied in a great number of healthy persons and in patients with a wide variety of internal diseases, especially vascular and thrombo-embolic disease over the past years. The incidence of enhanced platelet aggregation in healthy subjects is increasing with age. In diabetes mellitus the incidence of enhanced platelet aggregation was greater than in age matched controls, and there was another 10% increase in diabetes of more than five years’duration. This finding is in good correlation with the progression of diabetic angiopathy with the duration of the underlying disease. A very high incidence of enhanced platelet aggregation and only rare cases with normal (=missing) aggregation were observed in patientspriorto thromboembolic disease like myocardial infarction and arterial or venous thrombosis.Studies with a recently developed aggregometer revealed the importance of time dependant changes of platelet morphology (primary shape changes) upon the shape of aggregation curves in spontaneous aggregation (PAT III) and ADP-, collagen- or epinephrine-induced aggregation. If the various aggregation tests are being performed at the right time after blood sampling, they will yield very similar results.
