SummaryAs recommcndcd by the proper authorities, the prothrombin time results for patients under oral anticoagulant therapy arc reported by our laboratory in International Normalized Ratios (I.N.R.).For the calculation of the INR-values we use a calibration factor, the International Sensitivity Index (I.S.I.), provided by the manufacturer of the thromboplastin reagent in use.Since standard reference plasmas with known INR-values are not yet available an«in-house»ISI calibration appears to be a useful internal quality control procedure.The imprccisions inherent to the use of INRvalues are almost negligible in clinical practice.Certainly they do not counter balance the overall advantage that all laboratories despite different thromboplastin reagents, can use identical therapeutic ranges for oral anticoagulation intensity.

SummaryMacleod’s or Swyer-Jamcs’syndrome (MLS) is characterized by abnormal hyperlucency of one lung or lobe, with the hyperlucent lung of normal size or smaller than the unaffected lung.Although most patients with MLS remain asymptomatic, they can sometimes present with a highly variable symptomatology. In this report we describe three patients with MLS of whom one presented no symptoms, the second an hemoptysis, and the third an unilateral pulmonary edema. These observations stress the important clinical heterogeneity of the Macleod’s syndrome.The diagnostic criteria of MLS arc reviewed, and a strategy to unravel the often difficult problem of unilateral lung hyperlucency is proposed.Acta Clin. SummaryThis case report illustrates the usefulness of three scintigraphic techniques in the diagnosis of diffuse interstitial soft tissue calcification and the pre-operative localization of the causative Parathyroid adenoma in a patient presenting with uremia and hypercalcemia. Diffuse uptake fry the lungs and the stomach was noticed on am Technetium-pyrophosphate bone scintigraphy. A Gallium-citrate lung scan was in agreement with interstitial lung pathology whereas chest X-rays remained normal. The preoperative localization of the cellular part of a largely cystic parathyroid tumour was done by means of a:201Thallium minus Wm Technetiumpertcchnctate substraction scan of the neck area.
