SummaryWe report a case of endocarditis due toLactobacillus plantarum,occuring on a mitral valve, as a complication of obstructive cardiomyopathy. The route of entrance is maxillary osteitis of a dental origin.Endocarditis due toLactobacillusis a rare infection. Its major characteristic is an occasional resistance to treatment with /j-lactamines. In our case infection recurred in spite of chemotherapy with ampicillin (6 weeks) and gentamicin (14 days). A new treatment with penicillin and gentamicin for 6 weeks resulted in cure. We ascribe the initial therapeutic failure to a moderate sensitivity of the strain to /3-lactamines (M.I.C. 1,25 /ig/ml for penicillin G). We have not observed any tolerance to/5-lactamines.Endocarditis due toLactobacillusshould be treated with a combination of a /Mactamine with an aminoglycoside, in the event of the isolated strain being little sensitive to /3-lactamines.

SummaryThe case of a patient with bilateral gynecomastia of 6 years duration is reported. The endocrine profile (high oestrogens, low testosterone and normal gonadotropin levels in plasma) was secondary to an abnormal oestrogen production from a testicular tumor, clinically not detectable but revealed by echography. The tumor was developed from the interstitial Leydig cells (leydigome) and showed no cytologic sign of malignancy; there were no detectable metastasis. After surgical removal of the testis with the tumor, a state of hypergonadotrophic hypogonadism developed, explaining the persistance of the gynecomastia and the moderate elevation of plasma oestrogens. It is probable that pre-existing testicular lesions are responsible for the persistance of the hypergonadotrophic hypogonadism in the postsurgical evolution.

SummaryDiabetic autonomic neuropathy is a poorly defi ned neurologic entity with morbid and occasional lethal consequences. Cardiovascular effects of the diabetic autonomic neuropathy may include resting tachycardia, postural hypotension and painless myocardial infarction. The involvement of cardiovascular reflexes (beat-to-beat variation during breathing of Valsalva manoeuvre) assesses a cardiac parasympathetic dysfunction.The occurrence of a postural hypotension implies an orthosympathetic damage. We analysed the incidence of autonomic neuropathy in a group of totally insulindependent diabetic patients (mean circadian C peptide level: 0.04 + 0.003 pmol/ml, mean±SEM) and in a group of diabetic patients, with a residual insulin secretion (mean circadian C peptide level: 0.80±0.09 pmol/ml). 44 % of all the patients had up to 4 cardiovascular abnormal tests.Postural hypotension was observed in 11 % of the subjects. We found a significative correlation between the autonomic neuropathy and the duration of diabetes in subjects of group I (p<0.01). No correlation between autonomic neuropathy and C peptide level or mean glycosylated hemoglobin was observed. The high incidence of autonomic neuropathy and the poor prognosis of this diabetic complication justifies its systematic research in all type I and type II diabetic patients.

SummaryA new generation of specific and sensitive substrates for enzymes has been developed; they consist of a peptide moiety and a chromophoric leaving group, usually p-nitroanilide. The rate of release of p-nitroaniline from this type of substrate is proportional to the enzymatic activity, and can be measured spectrophotometrically from the increase in the absorbancy at 405 nm per unit of time. The specificity of these chromogenic substrates is determined by the nature and the sequence of their amino acids. Possible applications of these substrates in the field of blood coagulation and fibrinolysis include the monitoring of heparin therapy, oral anticoagulant therapy and thrombolytic therapy. The clinical value of a number of these chromogenic substrate assays is still under investigation.

SummaryTwo cases of moderate congenital factor VII deficiency arc reported. Both patients were admitted for surgery. They had never experienced any abnormal bleeding tendency. Coagulation screening showed a prolonged prothrombin time and a normal activated partial thromboplastin time. Specific assay of factor VII showed a moderate deficiency in both patients and in some of their family members.Preoperative replacement therapy was administered only in one case. In both cases there was no excessive blood loss during and after surgery.In congenital factor VII deficiency, surgery can be performed without any problem and routine preoperative replacement therapy can be avoided.

SummaryWe report the case of a 74-year-old-man, who developed severe progressively increasing dyspnea, during the 24 hours following a staging bipedal lymphography for non-Hodgkin lymphoma.The diagnosis of pulmonary thromboembolism has been established.The authors discuss the differential diagnosis with pulmonary oil embolism and suggest periodically passive exercise of the legs during lymphography in patients with high risk of venous thromboembolism, or even prophylactic heparinisation during the procedure in certain cases with extensive compression on larger veins.

SummaryAn unusual association of duodenal lymphoma and an acute myeloblasts leukemia is reported despite the fact that the patient had received no prior chemo-radiotherapy. The hypothesis of a defect at the level of stem cells causing the formation of two abnormal clones is suggested in order to explain this exceptional association.
