SummaryCaptopril in association with furosemide was used to treat seven patients with intractable severe hypertension. All patients were unresponsive (dias tolic blood pressure higher than 105 mm Hg) to tritherapy (diuretic, betablocker, and vasodilator). An important fall in blood pressure was observed in all cases, and in four patients permitted a complete control of blood pressure, with only low doses (less than 300 mg/day) of the drug. This good effect was already present after one month of treatment, and persisted unchanged for as long as 18 to 26 months. Plasma renin activity increased and remained elevated after one year of treatment.Side effects were encountered in four patients: skin rashes in one, moderate taste disturbance in two, and one episode of orthostatic hypotension Which disappeared with furosemide withdrawal. None of these side effects caused the interruption of captopril. The careful follow-up of serum creatinine and creatinine clearance during one year of therapy in the seven patients confirms that a long-term blood pressure reduction by captopril in severe hypertensives with altered renal function may be obtained without further decrease of the glomerular filtration.

SummaryThe case histories of two patients with lymphoid interstitial pneumonia (LIP) are presented. In one of those patients a malignant lymphoma was diagnosed. The discussion deals with the most important characteristics of LIP and particularly emphasizes the difficulties in differential diagnosis. Lymphocytic interstitial infiltration is best considered as a prelymphomatous status and is frequently associated with other features of immune dysregulation.

SummaryA collaborative study of the first Belgian Standard for Factor VIII assays has been carried out. It involved participants working in five University Hospitals, who were asked: 1) to calibrate the Standard in VIII: C International Units; 2) to measure the VIII: C and VIII R: Ag levels in numerous samples of fresh normal plasma, against the Belgian Standard; and 3) to define whether the Belgian Standard could be used for VIII R: RCF measurements.One VIII R: Ag Unit was defined as the amount of antigen present in that volume of average fresh normal plasma containing 1 VIII: C International Unit.The calibration of VIII: C in International Units disclosed important differences between the five laboratories, probably due to the difficulties encountered in the use of the International Standard. When tested against the 2nd International Standard, the Belgian Standard was calculated to contain 0.99 VIII: C I.U. and 1.08 VIII R: Ag/vial; when tested against average fresh normal plasma taken as 100%, the Belgian Standard was shown to contain activities equal to 78 % for VIII: C and 85 % for VIII R: Ag.Though VIII R: RCF assays are feasible with the Belgian Standard, its direct use as a reference material is not recommended, as the precise VIII R: RCF potency and stability are presently unknown. However, the Belgian Standard could be used indirectly for the calibration of the local plasma pools utilized for VIII R: RCF measurements.The use of the first Belgian Standard for Factor VIII assays in the clinical laboratories of our country is recommended by the authors.

Their technique confirms the high betaglucuronidase content of malignant plasmacells, as stated by other investigators and gives a«quantitative»expression of the difference in (cytochemical) enzyme content between normal and malignant plasmacells.This method is helpfull in the diagnosis of myeloma.

SummaryWe review the pathophysiology, etiology, clinical manifestations and treatment of the superior vena cava syndrome (SCS). A new etiology of SCS is described: the SCS caused by a central venous catheter which might become more frequent in the next years. The therapy of the neoplastic diseases which are responsible of 97 % of the SCS is discussed, as well as the introduction of chemotherapy as a first line treatment for small-cell anaplastic lung carcinoma.

SummaryThe authors summarize their experience after more than 2000 determinations of intrathyroidal iodine (ITI) by means of the X-ray fluorescence methode (XRF) and review the literature. This method explores the storage function of the thyroid, the so-called iodine pool with slow turnover rate, function that has hitherto been neglected because of lack of easily available means. The procedure is inocuous and the detector can readily be mounted on a conventional rectilinear scanner.The pattern of distribution of stable iodine within the gland and its qualifications have contributed to a better understanding of various thyroid disorders. Aside from its possibilities as far as the study of thyroid physiopathology is concerned, the method deserves its place in a routine clinical laboratory. Important information can be obtained, useful for diagnosis, treatment and prognosis in many instances, especially in hyperthyroidism and autoimmune thyroiditis. Altough it will not replace a radionuclide scan in most cases, it can serve as a substitute in those circumstances in which a radionuclide scan is not warranted or impossible to perform.

SummaryToxic shock syndrome is a recently described entity, probably mediated by a staphylococcal exotoxin. It is usually found in young menstruating women using vaginal tampons. The patient described here presents the classical features of the syndrome including fever, scarlatiniform rash, inflammation of the mucous membranes, hypotension, peripheral signs of shock, myalgia, gastrointestinal symptoms and signs of toxic encephalopathy. Muscle, lung, liver, heart and kidney are involved. Our patient showed renal failure apparently related to renal hypoperfusion. The classically observed hypocalcaemia was associated with high levels of calcitonin. Treatment with chloramphenicol, used as an empirical broad-spectrum agent, resulted in prompt recovery.
