SummaryThe effects of an a-glucosidase inhibitor, Acarbose (Bay g 5421) upon the circadian plasma glucose profile were studied in 7 diabetic patients in i hospital. All subjects were totally insulin-dependent as demonstrated by their low preand postprandial plasma C-peptide levels. The patients received Acarbose (5 days) or a placebo (4 days) in a randomized order.The mean plasma glucose levels, the M value and the MAGE index recorded during both periods were compared. Acarbose was given at doses of 200, 100 and 100 mg before breakfast, lunch and dinner, respectively. The doses of regular insulin (4/day) were daily adapted throughout the study in an attempt to reach the best possible glycemic control. Even though the insulin doses did not significantly differ during both periods, a significant dei crease of the mean plasma glucose levels was observed when the patients received Acarbose (8.2±0.4 vs 10.1±mmol/1; P<0.05). Moreover, the drug induced a significant decrease of postprandial plasma glucose measured at 10.30 am and 8.30 pm; the M value was also lower during Acarbose than during placebo (36±6 vs 45±6; P<0.02) while the MAGE index remained unaffected. Except for an increase in the number of hypoglycemic episodes, no side effects were recorded. Thus, Acarbose could represent an useful complementary drugt in the treatment of type 1 diabetes, essentially by lowering the postprandial rise of plasma glucose.

SummaryOur study describes the history of 6 patients with endocarditis, two of them with fatal outcome, caused by nutritionally deficient viridans streptococci, which could only be grown as satellite colonies on blood agar. Supplemented Schaedler broth proved to be superior to supplemented brain heart infusion broth for the isolation of these organisms from the blood. Deficient streptococci were also isolated from the blood of 6 non-endocarditis patients. However, only one blood culture was positive per patient and the isolates were not considered clinically significant.The literature of nutritionally deficient streptococci is briefly reviewed.

SummaryStarting from two cases of intestinal involvement in von Recklinghausen’s neurofibromatosis, one associated with obstructive jaundice and the other complicated by sarcomatosis transformation and bleeding, the authors report the frequency and the clinical features of digestive localizations. In a third case of neurofibromatosis, endocrine tumors were observed; this association is also discussed.In a series of 20 patients with neurofibromatosis 30 % presented visceral and 15 % digestive involvement. Among 4 cases of bleeding, two were due to digestive localization of the disease.

SummaryWe have employed combined parenteral nutri tion and chemoor radiotherapy in 36 cancer pa tients. Severe malnutrition prevented initiation 01 continuation of adequate antineoplastic treatmeni in 24 of them; the 12 others were to be treated by high-dose chemotherapy and autologous bone mar row transfusion.The infusion catheter is placed in superior ven£cava via the right internal jugular vein. The nutri ents, including lipids, are mixed under sterile condi tions in one disposable nutritive bag.The nutritional support permitted to 8 out of 1C patients to receive their first chemotherapeutic treatment without further delay; 12 of 14 patients could continue an antineoplastic treatment at ade quate doses despite severe malnutrition. Lastly the 12 patients treated with high-dose chemotherapy tolerated the treatment without alteration of nutri tional parameters.Infectious and metabolic complications were rare and always reversible. Four infections were catheter-related; only 2 septicemias have been ob served among 12 severely neutropenic patients. Two cases of hyperbilirubinemia were probably secondary to caloric overload.This study confirms that parenteral nutrition can be of considerable help in the management of selected medical cancer patients. But only prospective and randomized studies will precisely define its indications and benefits.

SummaryAuthors report on their experience with bronchoalveolar lavage in 207 patients with various pulmonary diseases. They confirm an increased number of lymphocytes and high protein concentrations in the lavage fluid of many patients with pulmonary sarcoidosis or hypersensitivity pneumonitis. Smoking tends to dampen the lymphocytic alveolar reaction associated with active sarcoidosis. Serum angiotensin converting enzyme activity (SACE) is often increased in sarcoid patients whereas it remains within normal limits in subjects with hyper sensitivity pneumonitis. Parallel measurement of lymphocytosis in the bronchoalveolar lavage fluid and of SACE may help to differentiate both diseases. Evidence will be also presented that bronchoalveolar lavage can be easily and safely used for the diagnosis of pulmonary siderosis, alveolar proteinosis and chronic eosinophilic pneumonia.

SummaryIn one female and one male patient, an auto-antiM antibody was detected during pretransfusion testing at 4°C. Neither of the patients had developed signs of cold agglutinin disease.The antibodies showed dosage effect, were pH dependent and belonged to the IgM class. SummaryAuthor’s personal experience after 100 transvenous liver biopsy is reported.A particular reference to sus-hepatic manometry and venography is given.Cirrhotic patients bleeding from esophageal varices or with ascites presents with a higher portal pressure than other cirrhotics. Moreover, cirrhosis has been diagnosed whenever pressure gradient was superior to 23 mmHg.This exam seems particularly relevant in cases of portal hypertension and is, of course, the only biopsy technique available when a transcapsular ap proach is contraindicated.

SummaryHematogenous metastasis is thought to be facilitated by the interaction of tumor cells with host platelets. The mechanism(s) by which such interactions facilitate metastasis is unknown. We have explored the possible relationships between arachidonic acid metabolism in tumor cell-induced platelet aggregationin vitroand tumor cell-platelet-vessel wall interactionin vivo.Our working hypothesis has been that the primary tumor, circulating tumor cells and membrane vesicles spontaneously shed from tumor cells alter the balance between thromboxane, a proaggregatory agent, produced by the platelet and prostacyclin, an antiaggregatory agent, produced by the vascular endothelium.In vitrowe have demonstrated that prostacyclin inhibits aggregation of human platelet rich plasma induced by a variety of murine and rat tumors. Tumor cell induced-platelet aggregation occurred concomitantly with generation of thromboxane A2and thromboxane synthetase inhibitors reduced platelet aggregation although much less effectively than did prostacyclin.In vivowe have demonstrated that both endogenous and exogenous prostacyclin reduce lung colony formation from tail-vein injected tumor cells. Nafazatrom, a drug believed to stimulate endogenous production of prostacyclin by the vascular wall, inhibited formation of lung colonies from tail-vein injected tumor cells and spontaneous pulmonary metastases from subcutaneous tumors.Platelet aggregating activity has been found in tumor cells, tumor cell shed membrane vesicles and in material extracted from tumor cell membranes.The identity of the platelet aggregating activity is not yet clear. We propose that one principle responsible for tumor cell-induced platelet aggregation and possibly for tumor cell procoagulant activity is a tumor cell-derived cathepsin B-like cysteine proteinase. We have shown that there was parallel inhibition of 15 16 amclanotic melanoma cathepsin B activity by selective inhibitors of cysteine proteinases. In addition, we have used papain, a plant cy steine proteinase, with 100% active site and active site groove sequence homology to cathepsin B as a mimicking agent for cathepsin B. Papain-induced platelet aggregation and papain cathepsin B-like enzyme activity were inhibited in parallel by selective cysteine proteinase inhibitors. Platelet aggregation induced by papain occurred concomitantly with generation of thromboxane A2and was inhibited by prostacyclin.Collectively our studies indicate that selective manipulation of arachidonic acid metabolism could alter the tumor cell platelet-vessel wall interactions to decrease metastasis.

SummaryA female patient with multinodular goiter was treated with methimazole (MMI) for thyrotoxicosis. After 3 weeks, she developped sore throat, pruritus, arthralgias, facial oedema and fever. Biological data were suggesting a scrum sickness. To our knowledge, this is the first published case report of MMI-induced serum sickness.Acta Clin Belg. 38, 1. 68–9The thioamide antithyroid drugs, propylthiouracil (PTU) and methylmercaptoimidazole (methimazole or MMI), share several side-effects, including skin rashes, neutropenia, agranulocytosis, jaundice and connective tissue disease-like syndromes (1). Vasculitis and serum sickness were also reported with PTU (1, 2). We describe here a case of serum sickness following the administration of MMI.
