The physician who is entrusted with a large quantity of resources for the treatment of ARF/MOSF has a social responsibility for optimizing the treatment, auditing the outcome, and where possible, preventing the illness. Best treatment includes not only modern techniques but also efficient organization and leadership of the ICU team. Audit is an individual responsibility, may be fruitfully pursued on a multicenter basis, and is a cost-containment discipline. Audit of the St. Thomas' Hospital ARF program has resulted in a predictive scoring formula. This is used to illustrate a cost-effective analysis related to disease severity. Prevention of ARF/MOSF is a challenge to emergency services. There may also be a considerable opportunity to prevent ARF in high-risk surgical patients. A randomized study carried out at St. George's Hospital illustrates the health gains which could result.

Cyclosporine and related compounds have dramatically improved the short-term success of solid organ transplantation. This paper outlines the pathophysiologic mechanisms of the nephrotoxicity of these compounds, highlighting relevant clinical and experimental work. It is hoped that as further understanding of these mechanisms evolves, rational therapeutic strategies will be available.

摘要:
A role of cytoplasmic free calcium [Ca2+]iin hypoxia-induced proximal tubule damage has been proposed. To further investigate the role of[Ca2+]iin mediating hypoxic proximal tubular injury, a video imaging technique has been developed in which [Ca2+]ican be measured simultaneously with propidium iodide (PI) staining of nuclei as an index of hypoxia-induced membrane damage. Hypoxia in rat proximal tubules is associated with a significant rise in [Ca2+]iwhich precedes evidence of membrane damage as assessed by PI staining. This rise in [Ca2+]iactivates calpain, a Ca2+-dependent cysteine protease, and constitutive nitric oxide synthase (NOS), the Ca2+-dependent form of NOS. Inhibition of either calpain or nitric oxide synthase provides marked cytoprotection against hypoxic proximal tubular injury. These observations are consistent with the early rise in [Ca2+]iinitiating hypoxic injury by activating NOS and calpain.
