Recommendations for triglyceride, high-density lipoprotein cholesterol and low-density lipoprotein (LDL) cholesterol measurements are influenced by the lack of true reference methods for these components, the need to calculate LDL cholesterol concentration from two or three primary measurements and by the inclusion of the contributions of several other atherogenic lipoproteins in the LDL cholesterol estimates.

This paper presents an overview on the project of the International Federation of Clinical Chemistry Committee on Apolipoproteins to standardize the measurement of apolipoproteins A-l and B. The aims of this study, performed in collaboration with the major companies manufacturing instruments and/or reagents for apolipoprotein determination, were to select and evaluate reference materials suitable for consideration as international reference materials and to standardize commercially available methods by a common accuracy-based calibration. Preliminary results indicate that uniformity of calibration results in comparability of apolipoprotein A-l and B values on serum samples.

This paper reviews the main apolipoprotein (apo)A- and apoB-containing lipoproteins, as defined by their apolipoprotein composition, identified so far. Methods of apoA- and apoB-containing particle quantification are briefly described. Clinical results obtained with these methods are reported.

New research firmly establishes the reverse cholesterol transport pathway, which carries cholesterol through the high-density lipoprotein, as significant in normal cholesterol homeostasis. Modulation of this pathway by transgenic and pharmacological techniques indicates potential therapeutic functions for increased high-density lipoprotein levels in atherosclerosis.

Several! recently developed methods assessing triglyceride metabolism have resulted in clearer definitions of distinct disorders of triglyceride metabolism. Beyond the basic lipid profile (plasma inspection, chemical determinations of cholesterol, triglycerides and high-density lipoprotein cholesterol) a judicious use of simple to complex methods allows the clinician and researcher to arrive at the phenotypic, etiologic or genotypic diagnosis of most such disorders.

Mass spectrometry (MS) is a powerful technique for solving otherwise intractable problems. We have outlined some of the recent developments in gas chromatography MS, high-performance liquid chromatography MS and Tandem MS, which should prove useful in both qualitative and quantitative lipid research. Two emerging techniques are introduced: capillary zone electrophoresis MS and supercritical fluid MS.
