The purpose of this study was to assess how patients with burns perceive their physical, psychological, social, and economic status since their burn injuries and to examine the question, “Does burn severity or age at the time of injury affect the patient's perception of his or her quality of life after burn injury?” A 25-item questionnaire was mailed to 1837 former patients who had been hospitalized in our adult burn center from 1975 to 1987, and 245 (13.1%) completed questionnaires were returned. TheX2test suggests that the patient sample overall did not experience a change in perceptions after burn injuries. However, when the adult group is divided by burn severity (grouped into low and high percent of burn) and by age at the time of injury (grouped into young and old), both the young and the older persons with high burn severity identify impairments in their quality of life after burn injury.

A prospective randomized study was undertaken to compare compliance efficacy and cost of the elastic nylon pressure garment (Jobst Institute, Inc., Toledo, Ohio) with the cotton elastic pressure garment (Tubigrip, SePro Healthcare Inc., Montgomeryville, Penn.). Of 110 patients enrolled, 54 received Jobst pressure garments and 56 received Tubigrip pressure garments. Time spent in pressure-therapy garments was the same for both groups. Comparable clinical results were achieved with either Tubigrip or Jobst garments. A significantly greater percentage of patients were compliant with Tubigrip pressure-garment therapy than with Jobst pressure-garment therapy. The cost of the Tubigrip garments was significantly lower than that of Jobst garments. These data suggest that the use of elasticized cotton pressure garments results in significantly better patient compliance, a lower cost, and equal therapeutic efficacy when compared with the elasticized nylon pressure garments.

Stridor is a common problem in patients with burns after extubation, especially in children and in those with facial burns or airway injuries. The usual treatments for severe respiratory distress, such as reintubation and tracheotomy, carry substantial risks for patients. We report our successful treatment of severe postextubation stridor in a 7-year-old patient with burns with the administration of heliox in addition to more traditional therapies, review the literature about die use of heliox in postextubation stridor, and suggest cases in which its use may prevent die need for more hazardous interventions.

Severely burned patients exhibit a postburn hypermetabolic response which, with the most severe burns, can double the patient's metabolic rate. We report on a 54-year-old man who was on long-term ibuprofen administration for treatment of arthritis before sustaining a 38% total body surface area burn. This patient failed to demonstrate the normal hypermetabolic response. The possibility that the ibuprofen administration prevented the hypermetabolic response is discussed.

A specific program in practice for community reintegration of the patient with burns is described. It has been demonstrated that a gradual transition for the patient with burns between the acute care setting and active participation in family, leisure, and vocational roles enhances psychosocial and functional recovery. A systematic process of community reintegration within a rehabilitation setting can facilitate this transition for patient and family through Community Skills Training with a therapist and Community Skills Practice before the patient assumes outpatient status.

Biobrane temporary biosynthetic skin substitute has been well demonstrated in the treatment of superficial and deep wounds. We have used this product nearly 1000 times. It has previously been shown to be ineffective for use in areas with convex or concave topographies or in areas along joint surfaces. The following study covers an attempt to adapt this dressing for use hi these difficult areas. Ninety-eight applications of meshed Biobrane temporary wound dressing were placed in anatomic locations, which ranged from the head to the foot, with an 86% success rate.

The disabled are handicapped only to the extent that they are prevented from being normal. The process of normalization is hampered by physical barriers in the home that make life difficult for disabled persons. Staff can be trained to eliminate these architectural barriers in the home, focusing primarily on accessibility, the bathroom, the bedroom, and the kitchen. Completion of the barrier-free design of the home should provide a cost-effective means for improving the quality of life for disabled persons and their families.

A review of 11 patients who sustained burns that were complicated by limb amputation was completed to determine their eventual ability to use prosthesis. Amputations included six below-elbow, four above-elbow, three below-knee, and three above-knee amputations. Ten of the 11 patients (91%) had open wounds on the stump limbs and nine patients (82%) required skin grafting procedures on the amputated limbs. Delays in prosthetic fitting because of continued surgeries, open wounds, skin grafts on the stump limb, and breakdown of the stump were identified. However, eight of these patients (73%) were eventually able to wear prosthetic devices. Fisher's exact test was used to test the influence of the number and site of the amputations and skin grafting on the stump on successful prosthesis use. None of these tested items were found to be significant. The results demonstrate that most patients with burns who require limb amputation can achieve successful prosthesis use.

The purpose of this study was to develop a discharge planning index for use in a pediatric acute burn unit. The study sample consisted of 59 health professionals from a pediatric burn hospital who had worked at least one year with children with acute burns and their families in discharge planning. With the use of the Delphi technique, a three-round survey was conducted. A Pediatric Acute Burn Discharge Planning Index that consists of 75 items was developed on the basis of the items that received a mean score of 5.0 or greater in the third survey. The data were further analyzed with Spearman's rank correlation coefficient to compare the top 23 rank-ordered items rated by the nursing department with those rated by the other departments surveyed. The rank-order correlation showed a positive correlation between the nursing department's and other departments' responses. These results indicate that all health team members agree on the importance of assessing the items listed on the derived index before discharging a pediatric patient with acute burns.
