摘要:
The Board of Health of the Commonwealth of Virginia has an outdated sanitary code for its public hydrotherapy and swimming pools. The code is restricted to pools in hotels and other lodging places. The absence of modern regulations for public hydrotherapy and swimming pools has permitted serious deficiencies in pool maintenance, which are highlighted in this report. The most notable of these deficiencies was the presence of high levels of bacterial contamination that could predispose to infection in the water of one public hot tub. The results of this study indicate that the Virginia Board of Health sanitary code for pool water must be revised immediately and should include all public hydrotherapy and swimming pools. Other states and communities may want to assess their codes for swimming pools and hydrotherapy tubs to avoid deficiences that could be detrimental to public health.

One hundred burn care facilities in the United States were surveyed to determine the present use of hydrotherapy in burn care. It was found that 92% of the burn units that were polled practice this treatment in some form. There was no consensus as to how the procedure is carried out, how long it takes, how often it is done, or who does it. Most burn facilities that were contacted use a disinfectant solution for hydrotherapy, and the procedure is carried out once a day for 30 minutes or less. In most of these facilities, nurses perform the procedure. Although immersion therapy continues to play an important role in most burn care facilities, spray or shower techniques are also incorporated into many programs.

Pulse oximetry is a noninvasive method of measuring arterial oxygen saturation. The value of oximetry in patients with burn injuries has been questioned because of a theoretic inaccuracy in the presence of carboxyhemoglobin. We studied pulse oximetry in 27 intubated patients with burn injuries to determine the accuracy of the method and then to determine whether oximetry could replace indwelling catheters presently used for arterial blood gas analysis. Oximeter and arterial blood gas saturation data correlated closely, with a coefficient of 0.820. The pulse oximeter predicted “adequate” ventilation in 78% of patients with a readout of 99% or above. The arterial Po2was ≥90 torr in 90% of patients with oximetric readouts ≥98% and in 10% of patients with readouts <95%. Pulse oximetry is an accurate adjunct in the management of patients with burn injuries and in addition provides continuous real-time data not available with arterial blood gas sampling.

Injuries are a leading cause of childhood mortality and morbidity in the home, particularly in children less than 6 years of age. In an effort to prevent childhood injuries, the Hennepin County Burn Center began a joint venture with a public health agency to reduce home hazards for children less than 6 years of age who were treated at the burn center. Children were referred to a public health nurse for a home safety assessment. During the initial visit, child hazards were identified and recommendations were made for reducing injury risk. Parental compliance with recommendations was evaluated during a second home visit approximately 1 month later. Home safety assessments were completed in 21 homes, and a total of 131 recommendations were made. Burn prevention recommendations accounted for 43.5% of the total recommendations, poison control recommendations accounted for 36.6%, and other injury control recommendations accounted for 19.9%. Parents complied with 43.5% of the total 131 recommendations. Burn prevention recommendations had a compliance rate of 42%. Poison control recommendations had the highest rate of compliance at 58%. Other injury control recommendations had a compliance rate of 19%. The program had a positive effect on reducing home hazards. It was an appropriate response by practitioners already involved in the care of children, many of whom are at risk of injury.
