Nursing staff turnover in a burn center is costly in both dollars and staff stress. A better understanding of predictors of tenure would help to lessen this problem. The Strong- Campbell Interest Inventory, a multifaceted questionnaire used to predict vocational interest, was administered to three groups of nurses: burn nurses (42), surgical intensive care nurses (26), and rehabilitation nurses (22). The study showed that there were no tenure differences between the burn nurses and surgical intensive care nurses and that burn nurses differed from rehabilitation nurses only on the investigative theme score IP < 0.05). Likewise, no relationship was found between length of employment and tenure in burn nursing

Production of (β -lactamases is an important mechanism in bacterial resistance to penicillin-class antibiotics. Clavulanic acid a (β -lactamase inhibitor, has been combined with amoxicillin lAugmentin) and ticarcillin (Timentin) to form a combination with a broader spectrum than the individual components alone have. These combination drugs have been tested in vitro for activity against bacterial isolates recovered from burn patients. In both cases, the antibiotics plus clavulanic acid showed broader spectra of activity than did the antibiotics alone. There were considerable differences among the various genera and species tested, however. These drug combinations may have some indication for the treatment of burn infections but would have to be used selectively

Retrospective analyses of 1,271 burn patients admitted to the University of Alabama (UAB) burn center from 1972 to 1981 showed the following trends in demographics, morbidity, mortality, and length of hospitalization. Patients aged 15 to 55 years with 60% or less body surface area (BSA) burn had a 93.2% survival. Of the 1,271 patients, 265 (20.9%) died. The mean age and percent BSA burn in patients who died were significantly higher than in those who survived. Complications related to mortality included pneumonia, septicemia, bacteremia, and respiratory burn. Preexisting conditions related to mortality included alcoholism, diseases of the heart and circulatory system, and mental disorders. The average length of hospitalization decreased significantly from 1972 to 1981, probably because of the decrease in burn size as well as the introduction of the enzymatic fast-graft technique for rapid wound closure

Several incidents of fourth-degree electric burns to the quadriceps muscles have been treated in our regional burn unit. Suggested therapy for exposed muscle is maintenance of a moist environment to prevent desiccation of muscle tissue and limiting the amount of physical stress to the involved area to avoid rupture of the muscle fibers. This method generally allows preservation of the muscle, but frequently leads to decreased function and a prolonged rehabilitation course following wound healing.In five patients, total rehabilitation and restoration of normal function involving herniated quadriceps muscles were achieved through aggressive physical therapy and early surgical excision following electric burn. At the time of injury, tetanic depolarization of the involved muscles by electric current resulted in complete and maintained extension of the leg, but complete and normal range of motion and function returned within 27 days

Scalds are a common cause of thermal injury in all age groups. An analysis of California Burn Registiy data compiled during its first four yeare of operation showed that 618 patients had scald injuries. Of those categorized in this report, 180 (22%) had tap water scalds and 624 177%) had scalds due to other causes (average TBSA burns of 15.1 ± 1.1% and 11.2 ± 0.5% respectively). Most of the scalds occurred in the home. Twenty-one patients (2.6%) died as a result of scald injuries, usually from tap water. Prevention efforts, especially for those at greatest risk, are discussed

Obstructive sleep apnea (OSA) is defined as cessation of airflow at the nose and mouth due to complete occlusion of the upper ailway at the orophaiyngeal level during sleep. This results in episodes of hypoxia and sleep fragmentation, producing daytime symptoms of excessive sleepiness, abnormal behavior, learning problems, and failure to thrive. OSA is often associated with underlying conditions that narrow the upper ailway. In three children with late complications of burns to the head and neck who had symptoms of OSA, sleep studies revealed severe OSA associated with profound oxygen desaturation during each episode. All responded to treatment. Although an uncommon complication of burns, OSA is important to recognize. The symptoms can easily be misinterpreted as those of psychological disturbance following bums. A history of snoring interrupted by pauses, the hallmark of OSA, will identify those children who warrant further investigation in order to institute appropriate therapy
