After completing enterostomal therapy training this writer has discussed with the agency's Director of Health Services the role of an ostomy specialist within the Visiting Nurse Association of San Francisco. The position of Enterostomal Clinical Nurse Specialist within this agency is still in planning and implementing stages. Over three months, sixty percent of client visits have been to ostomy patients. The majority have been over the age of sixty-five. Special instruction and education was required by all patients and approximately ninety percent required ostomy equipment changes to improve their ostomy management. It is apparent that these patients did not receive adequate instruction in the hospital and that hospital personnel were unaware of how to properly use and recommend ostomy equipment. Obviously these patients were not independent in their ostomy care at the time of hospital discharge or able to psychologically deal with the full impact of surgery. By providing skilled nursing care and instruction, ninety-two percent of the ostomy patients are independent now in their ostomy care and many have resumed former interests and activities.
