Adenylate kinase activity was found in 32 of 34 samples of cerebrospinal fluid (CSF) from 21 patients with stroke and seven patients with global cerebral ischaemia (GCI). The light absorbance values of the spectrum 400–650 nm revealed the scanty occurrence of haemoglobin products in the CSF in some patients. No correlation was found between the absorbance values at 415 nm, reflecting oxyhaemoglobin, and the adenylate kinase activities. Thus, a main contribution to the adenylate kinase activity in CSF by leakage of this enzyme from erythrocytes could be ruled out. Instead increased leakiness of the brain cells, having an impaired metabolism due to insufficient supply of oxygen and glucose, was the most plausible cause of the findings. The quotient between the adenylate kinase activity and the light absorbance at 415 nm seemed to reflect the extent of ischaemically deranged brain tissue in the GCI patients, while the CSF‐lactate values were not correlated to the clinical outcome. The neuropathological findings in two cases of irreversible hypoglycemic brain injury are described. A 26‐year‐old diabetic man injected insulin without adequate food intake and died after 2 months in coma. An 84‐year‐old nondiabetic man accidentally received 10 mg of glibenclamide and died after 3 months in relatively superficial coma.In the first case, an extensive necrotizing injury with gliosis was present in the cerebral cortex with temporal preponderance, as well as in the amygdalae and hippocampus. Lesions were also present in the putamen and caudate nucleus whereas the globus pallidus and thalamus were less severely destroyed. The distribution of the lesions was therefore somewhat different from that commonly seen in hypoxic‐ischemic brain injury, which, together with some previously published data, suggests some difference in the pathogenesis of hypoglycemic vs. hypoxic‐ischemic brain injury.In the second case only a slight loss of cortical neurons with secondary gliosis could be attributed to the hypoglycemic insult. At autopsy, a 72‐year‐old white male with chromatin‐negative Klinefelter's syndrome, mental retardation, psychotic behavior, and carcinoma of the lung was noted to have right unilateral focal cerebral and cerebellar megalencephaly with broad gyri, a cytoarchitectually abnormal cortex, and thickened cerebral vessels. Literature about the neuropathology of Klinefelter's syndrome and several cases of other chromosome aberrations (XYY and XO syndromes) are reviewed. Use of subarachnoid/intraventricular administration of hyaluronidase in the treatment of 15 cases of cranial arachnoiditis which occurred as a complication of tuberculous meningitis is reported. Eleven of these cases had communicating hydrocephalus and four had optochiasmatic arachnoiditis. An average of 4.5 injections were administered at weekly to fortnightly intervals. These patients had been followed‐up for a mean period of 6.1 months per patient. Serious side effects such as flare‐up of the underlying meningitic process, miliary spread of tuberculosis, convulsions or allergic reactions were not observed. Improvement of sensorium and/or neurological deficit was observed in 14 of the 15 cases. A clinically comparable group of 15 cases subjected to shunt surgery (14) and ventricular drainage (1) were considered as “control”. This medical method of treatment appears to be superior to shunt surgery and offers a simpler alternative line of management of cranial arachnoiditis/hydrocephalus complicating tuberculous men

Lisuride 1.2–4.8 mg daily was given to 10 patients with severe Parkinsonism for up to 9 months. All had been taking bromocriptine and eight had been taking levodopa combined with carbidopa. Total replacement of bromocriptine by lisuride was achieved in every case, but partial or total levodopa replacement was possible only in five patients. Lisuride 1 mg has approximately the same antiparkinsonian activity as bromocriptine 15 mg or levodopa 250–500 mg combined with carbidopa, but the duration of action of each dose is short, and gastro‐intestinal and neuropsychiatric side effects are common. 摘要:
The use of the method with examination of jitter and fiber density is mentioned. Some patients (one with polyneuritis, one with dystrophia musculorum progressiva and three with myasthenia gravis) are presented, with demonstration of the jitter (the variability of the interpotential interval).Jitter examinated in musculus extensor digitorum communis was prolonged (over 55 microsec) in patients with myasthenia gravis and dystrophia musculorum progressiva.
