ABSTRACT—Patients with transient ischemic attacks (TIA) were previously shown to have high plasma values of thiobarbituric acid‐reactive substances (TBA‐RS). To study whether these changes could be related to platelet activability. TBA‐RS was investigated in 24 TIA patients before and 24 h after 1 g aspirin, an inhibitor of platelet cyclooxygenase pathway. Baseline TBA‐RS values were significantly higher in TIA than in controls. Conversely, TIA patients had TBA‐RS values after aspirin similar to controls, suggesting that the increase of plasma TBA‐RS was not attributable to platelet hyperfunction. The evaluation of metabolic profile showed that patients with highest TBA‐RS had hypercholesterolemia, hypertriglyceridemia, and/or diabetes mellitus. ABSTRACT—The article reports on 3 patients suffering from muscular atrophy after radiotherapy of the para‐aortal lymph nodes for malignant testicular tumor without any sensory, bladder, or bowel disburbances. By neurophysiological examination, a lesion of the lumbal plexus and the peripheral nerves of the lower extremities were excluded. On EMG‐examination there were no giant motor unit potentials, as they can be found in anterior horn cell lesions. ABSTRACT—Nimodipine (BAY e 9736), a new dihydropyridine derivative, has been shown to reduce neurological deficits and mortality induced by acute cerebral ischemia in experimental studies. We investigated the effects of this calcium antagonist in patients with acute ischemic stroke through a randomized, double‐blind, parallel‐designed trial in which nimodipine was compared with placebo. Forty‐one of 54 screened cases were found to fulfil the inclusion criteria (sudden occurrence of a focal neurological deficit secondary to an acute ischemic event in the carotid area diagnosed after a complete neurological work‐up) and entered the study. Nineteen of them were treated with nimodipine (40 mg t.i.d. administered for 28 days) and the remaining 22 with placebo, given in identical tablets. In all patients the treatment started within 12 h after the onset of the symptoms. Course and intensity of the neurological deficit were evaluated by the Mathew Scale (slightly modified). Forty patients concluded the trial. Nimodipine was withdrawn in one case following the occurrence of a skin rash whose causative relation with the test drug could not be clarified. Altogether, however, nimodipine was well tolerated and no severe cardiovascular adverse reactions were observed. In terms of efficacy, the scores obtained by the Mathew Scale showed a higher rate of improvement on nimodipine than on placebo, thus indicating that patients receiving the latter drug did not fare as well as those receiving the test medication. ABSTRACT—The prevalence of myasthenia gravis (MG) in the counties of Hordaland and Sogn&Fjordane on January 1, 1984 was 9.6 per 100,000 inhabitants. Other autoimmune diseases were found in 11 out of 48 MG patients. The occurrence of autoimmune thyroiditis (5 patients, 10.4%) and systemic lupus erythematosus (4 patients, 8.3%) in the MG patients was clearly higher than that reported in the general population. Rheumatoid arthritis was found in 2 patients (4.2%). The autoimmune diseases were mainly recorded among the nonthymectomized MG patients. In addition to those with definite diseases of autoimmune nature, 3 other MG patients had thyroid antibodies and 1 had antinuclear factor without clinical evidence of autoimmune disease. ABSTRACT—Reports of tremor in progressive supranuclear palsy (PSP) have been infrequent; some authors indicate that tremor is never seen in PSP. Five patients with PSP and tremor are presented. Our series of cases and those reviewed in the literature suggest that tremor occurs in 12–16% of PSP cases. The rest tremor observed in 3 of our cases differed from the “pill‐rolling” and “parkinsonian tremor” described in previously reported cases by being slower in rate (3 per second or less). This slow rest tremor is rarely seen in Parkinson's disease (4–6 per second). Our experience suggests that, contrary to previous opinions, tremor may be the presenting complaint and a prominent finding in PSP and the presence or absence of tremor is of no value in the differential diagnosis of PSP from Park

ABSTRACT—Among the extraintestinal complications of Crohn's disease, neuropathy seems to be rare, and is only exceptionally reported in the literature. The authors present the report of a patient followed over a 12‐year period who developed polyneuropathy with a parallel course to Crohn's disease. There was no evidence of any other underlying condition. On one occasion, the patient developed a cutaneous vasculitis with fibrinoid deposits in the vessels. The search for circulating immune complexes was positive. The authors demonstrated increased intestinal permeability using the51chromium‐labeled ethylenediaminetetra‐acetate (51Cr‐EDTA) test. ABSTRACT—Glucose metabolic rate constants in patients with Alzheimer's disease were analyzed using dynamic positron emission tomography with [18‐F] fluoro‐2‐deoxyglucose (FDG). The cerebral metabolic rate of glucose (CMRG) was calculated using obtained rate constants, as well as the autoradiographic method. The half‐life and distribution volume of FDG between the blood and the brain tissue were also calculated from obtained metabolic rate constants. The most severely affected metabolic step was the phosphorylation of glucose in the parietotemporal regions. The distribution volume of FDG showed no remarkable deviation from normal controls, while the half‐life of FDG was found to be longer in the parietal and temporal regions. ABSTRACT—A 10‐year‐old right‐handed boy showed conduction aphasia with left‐ear verbal extinction (paradoxical ipsilateral ear extinction) after removal of a arteriovenous malformation in the left parietal lobe. Buccofacial and ideomotor apraxia were not observed. Recovery from aphasia was dramatic. Postoperative computed tomography (CT) and magnetic resonance imaging scans demonstrated damage confined to the left supramarginal gyrus invading the arcuate fasciculus. Together with recently reported cases of acquired fluent aphasia in children with CT‐verified left posterior lesions, this case seems to support the current view that both fluent and nonfluent aphasia that share many similarities with the symptoms and lesion localization associated with adult cases may exist

ABSTRACT—The lipid composition of white matter and myelin from the semioval centre was studied in autopsy material from cases with Alzheimer's disease (AD) (n= 11), vascular dementia (VD) (n= 7), and age‐matched controls (n= 11). In AD and VD the white matter content of phospholipids and cholesterol was reduced to 72–76% of the control values (P<0.01), the diminution of cerebrosides and sulphatides was more pronounced (55–69%) (P<0.001) while the concentration of gangliosides did not change significantly (87–90%). The myelin composition was the same in the 3 groups, suggesting that the white matter involvement is not caused by alteration of the myelin structure.
