AbstractIn a prospective study of neurological complications of coronary bypass surgery, detailed pre‐ and post‐operative bedside ophthalmological evaluation was undertaken in 312 patients. Post‐operative neuro‐ophthalmological complications developed in 80/312 (25.6%) patients and included: areas of retinal infarction (17.3%); retinal emboli (2.6%); visual field defects (2.6%); reduction of visual acuity (4.5%) and Horner's syndrome (1.3%). Neuro‐ophthalmological complications were not observed in a control group of 50 patients undergoing major peripheral vascular surgery. Ten of 75 patients reviewed at 6 months still had detectable neuro‐ophthalmological abnormalities, but functional disability occurred only in those with persistent visual field defects. Multivariate analysis revealed that extra‐coronary vascular disease, severe and prolonged duration of heart disease prior to operation, and large drop in haemoglobin level during surgery may predispose to neuro‐ophthalmologica

AbstractA newly recognised neurotoxic syndrome due to pyridoxine (B6) overdose is described. It is the largest series of B6 intoxication hitherto reported. A raised serum B6 level was present in 172 women of whom 60% had neurological symptoms, which disappeared when B6 was withdrawn and reappeared in 4 cases when B6 was restarted. The mean dose of B6 in the 103 women with neurological symptoms was 117 ± 92 mgs, compared with 116.2 ± 66 mgs in the control group. There was a significant difference (P<0.01) in the average duration of ingestion of B6 in the neurotoxic group of 2.9 ± 1.9 years compared with 1.6 ± 2.1 years in controls. The symptoms were paraesthesia, hyperaesthesia, bone pains, muscle weakness, numbness and fasciculation, most marked on the extremeties and predominantly bilateral unless there was a history of previous trauma to the limb. These women were taking a lower dose of B6 than previously described (1,2), which may account for the complete recovery within 6 months of stopping

AbstractCourse and possible prognostic predictors were studied retrospectively in 363 MS patients collected during an epidemiological study. The proportion of patients showing a primarily progressive course was 18%; 19% of patients of more than 10 years duration showed a benign course, and 8% of those of less than 16 years duration, a malignant course. Although benign cases were twice as frequent in females as in males, the difference between both sexes was not significant. A primarily or secondarily progressive course, a high number of functional systems involved and cerebellar signs occurring in the course indicated a poor prognosis whereas symptoms at onset, age at onset as such, and occupation were without predictive value. No predictors of a favourable course were found. Of the patients of 11–15 years duration, 17% were confined to a wheelchair, 40% were restricted to a walking distance of 500 metres or less, 61% were unable to work and 45% had urinary incontinence. AbstractNeutral protease activity was significantly elevated in the cerebrospinal fluid of patients with multiple sclerosis (MS) in exacerbation and in the acute phase of acute viral meningoencephalitis (AME) compared with that of MS in remission, amyotrophic lateral sclerosis or psychosomatic disease. Since in each relapse of MS, protease activity was higher in exacerbation than in remission, this activity may be one good marker of disease activity in MS. One hundred micro molar of FOY305, synthetic protease inhibitor, inhibitedin vitroincreased neutral protease activity in MS in exacerbation, which suggests the possibility of a clinical application of this protease inhibitor for MS.

AbstractThe serum of 8 patients with chronic inflammatory demyelinating polyneuropathy (CIDP) was tested using a chromium release cytotoxicity assay and cultured Schwann cells. Serum was obtained from all patients prior to treatment by plasma exchange, which was beneficial in some patients only. Those patients with high serum cytotoxicity (high chromium release) were those who showed a positive response to plasma exchange.

AbstractSkin potentials (SP) were evoked by peripheral nerve stimulation from the hands and feet of 41 and from the genital skin of 28 male, controls. The same methods were also applied in 10 functionally impotent cases, 32 diabetic impotent and 8 diabetic normopotent cases. The SP was easily obtained from all 3 sites in all normal subjects and in 10 functionally impotent cases. The SP recorded from the genital skin was either absent or abnormal in 53% of diabetic impotent cases with or without polyneuropathy, while the incidence of abnormalities encountered in the hand and/or foot recordings was 28%. In 8 diabetic normopotent cases the SP recorded from the genital and extremity skin were completely normal. SP recorded from the genital skin is a useful method of evaluating the autonomic‐sympathetic dysfunction causing impotence, especially in patients who may have autonomic disorders either located in lumbosacral spinal cord and its efferents, or generalized autonomic‐peripheral dysfunct
