ABSTRACTCerebral magnetic fields elicited by electric stimulation of peripheral nerves were studied in man. Responses were found over the Sylvian fissure at latencies of 95‐125 ms for both contra‐ and ipsilateral stimuli. ABSTRACTCerebral magnetic fields preceding self‐paced plantar flexions of the feet were studied with a SQUID gradiometer in 4 subjects. A slow magnetoencephalographic (MEG) shift was observed to begin as early as 1 sec before the movement. The shift changed its polarity between frontal and parietal areas. The MEG shifts preceding right and left foot movements were similar in shape, but their polarities differed at many recording locations. Simultaneous movements of both feet were preceded by shifts approximately equal to the sum of the shifts preceding the unilateral foot movements at the same recording location. The results suggest that the EEG and MEG shifts preceding foot movements are largely generated by tangential current sources on the mesial surface of the contralateral hemisphere around the motor representation area of the foo

ABSTRACTAs part of a clinical examination of 200 patients with pseudopapilloedema with and without verified optic disc drusen, neuroophthalmological examinations were performed in 158, neurological examinations in 117 and electroencephalography in 109 patients. Headache and suspected papilloedema were the most frequent reasons for referral. Headache was one of the complaints in 102 patients. Migraine was diagnosed in 22 and suspected in 3 patients. Epilepsy was present in 9 patients and, in addition, 1 patient had had treatment for convulsions in childhood. 2 patients had a pituitary tumour and 1 had a tumour in the hypothalamic region. Various other neurological disorders appeared in small numbers. Abnormalities in electroencephalograms (EEGs) were found in 36 patients. Progressive loss of central visual acuity unexplained by retinal pathology as well as bitemporal and homonymous hemianopic visual field defects and also evidence of papilloedema warrant a neurological examination in patients with optic disc drusen.

ABSTRACTThe urinary excretion of organic acids was examined in 50 cases with multiple sclerosis and in 50 age‐ and sex‐matched controls. The concentrations of the acids were related to creatinine. No differences were found for compounds such as glycolic acid, 2‐methyl‐3‐hydroxybutyric acid, 2‐ethylhydracrylic acid, 4‐hydroxyphenylacetic acid, suberic acid and many other acids. However, the mean excretion of 2‐hydroxy‐2‐methyl‐3‐butenoic acid was increased two‐fold in the MS group. 2 MS cases had a very high excretion of 3‐methylglutaconic acid, and another 6 cases had moderate elevations, which were fairly constant over a time period of several months. Moderate elevations were also noted in 2 healthy controls. 1 MS case had a very high excretion of 3‐hydroxyisovaleric acid. 7 MS cases, and none in the control group, had elevated excretion of adipic acid. Differences were also noted for lactic acid, succinic acid, aconitic acid and 3‐methyladipic acid.An oral dose of deuterium‐labelled acetate was given to one of the patients with high excretion of 3‐methylglutaconic acid. Deuterium was incorporated into this metabolite.3‐methylglutaconic acid, 2‐hydroxy‐2‐methyl‐3‐butenoic acid, 3‐hydroxyisovaleric acid and 3‐methyladipic acid are all potential isoprenoid metabolites. ABSTRACTThe purpose of this study was to evaluate the possible modifications of the plasma and urine short‐chain‐fatty acid (SCFA) patterns induced by treatment with valproic acid (VPA). Increased amounts of SCFAs in patients under VPA treatment may explain the presence of VPA‐induced hyperammonemia, toxic encephalopathies and rarer Reye‐like syndromes recently observed. For this reason we assayed SCFA levels in the plasma and looked for propionic acid in the urine of 10 epileptic patients to whom it was decided to add VPA to their previously unsatisfactory antiepileptic treatment. This was carried out prior to and during therapy with VPA. 5 of these patients developed toxic encephalopathy with hyperammonemia induced by VPA. ABSTRACTIn a double‐blind crossover trial of two 2‐week treatment periods, to which 18 patients with benign essential tremor were admitted, there was no statistically significant difference between progabide 11.4‐202 mg/kg x day (median 14.2 mg/kg x day) and placebo in tremor score and tremor amplitude. However, there was a definite placebo and period effect with reduction of tremor score and tremor amplitude during the second period of treatment compared to the first period of treatment, regardless of the treatment regime. ABSTRACT50 multiple sclerosis (MS) patients were evaluated, according to an objective weighting scale measuring neurologic deficit, and investigated with continous reaction time (CRT). Compared to a group of 105 controls, the reaction times of the MS patients were significantly delayed. The CRT method could correctly classify 80% of the controls and 72% of the patients. The CRT was especially sensitive for patients in the progressive phase of the disease and independent of dyscoordination.

ABSTRACTThe prophylactic effect of a 5‐HT uptake inhibitor, femoxetine, was compared with that of propranolol in a double‐blind crossover study of 6‐months duration. 29 patients commenced the experiment. 3 subjects withdrew because of side effects and 2 because the program was inconvenient for them. In the 24 patients who continued the study to the end, the periods of propranolol (administered 160 mg daily) and of femoxetine (given 400 mg daily) differed significantly from each other with respect to the attack frequency and headache index. A significant reduction in the use of medication relieving attacks was observed during the propranolol treatment as compared with the pre‐treatment period. The study showed that partial depletion of thrombocyte 5‐HT by a 5‐HT uptake inhibitor did not lead to a marked improvement in headache, contrary to what might be expected on the grounds of the 5‐HT hypothesi
