摘要:
The loss of lean body mass (muscle wasting) is initiated by cascades or events that precipitate increased proteolysis. Muscle wasting is stimulated by internal and external factors. Humorally related feedback loops stimulated by disease states, e.g., cancer, inflammatory myopathies, leukemia, or sepsis, and initiated as a response to systemic inflammatory response syndrome can cause a downward progression of events, resulting in chronic illness or even death. This article discusses the pathogenesis of muscle wasting, which is often referred to as cachexia.

摘要:
Developing quality continuing nursing education for contact-hour approval requires a basic understanding of the continuing nursing education (CNE) system and the application process through the National Association of Orthopaedic Nurses (NAON). The Approver Unit of NAON helps CNE providers (i.e., members, chapters, and corporate members) develop education activities and obtain contact hour approval. This article contains the latest information on CNE terminology and the application process for contact-hour approval through NAON.

摘要:
Fractures of the forearm in an adult may involve the ulna, the radius, or both, or one may be fractured and the other dislocated. Because the ulna and the radius are joined by the interosseous membrane, injury to one often also affects the other. In the same way, the humerus can be effected from injuries of pressure at the site of the elbow. It is always wise to x-ray the entire upper extremity in most upper-limb injuries. Whatever the injury is, the patient will be incapacitated for several months by loosing the use of his or her upper extremity.

PurposeTo describe the experience of verbal abuse in a large multihospital system and determine the relationship of verbal abuse with intent to leave the organization.DesignDescriptive correlational design using mailed questionnaires.SampleRandomized sample of 1000 nurses received questionnaire; 46% response rate, with a final useable sample of 461 surveys.FindingsOf the respondents, 91% had experienced verbal abuse in the past month. The physician was the most frequent source of verbal abuse, followed by patients, patient families, peers, supervisors, and subordinates. More than 50% of the sample did not feel competent in responding to verbal abuse. Amount of abuse and intent to leave were significantly related (r= .211,p< .01).DiscussionOrganizations must adopt zero-tolerance policies for professional verbal abuse. Education and coaching are needed to help nurses to improve their skills when responding to patient-to-nurse and professional-to-professional verbal abuse.

The PalmPilot Data Entry System with CaptureWare was developed to collect information, such as quality-of-life surveys and standardized orthopaedic clinical evaluations, on a PalmPilot personal digital assistant (PDA).The user downloads this information directly from the PDA into a Microsoft Access Database where software programs analyze the information. Before this technology was available, manual information entry into the database from paper forms was required. This necessitated the expense of data-entry personnel and resulted in a paper quagmire. The PDA system eliminates both these problems.This article discusses general PDA use in the medical field, then describes the PalmPilot Data Entry System experience for one firm, Orthopaedic Associates, Ltd. The firm is a community-based practice, with 4 orthopaedic surgeons who have used the system for 6 months. The PDA Data Entry System was inexpensive and easy to use and, overwhelmingly, generated positive response from patients, nurses, and physicians.

PurposeA role-delineation study was conducted by the American Society of Pain Management Nurses to examine the activities performed by nurses involved in pain management.DesignA survey was sent to nurses involved in pain management. The role-delineation survey consisted of 92 activity statements and was based on the Nursing Intervention Classifications. Frequency of performing each activity and importance of the activity were rated by respondents, and a Mean Activity Index score was calculated for each item.SampleAs part of the sampling strategy, a random sample of 200 nurse members of the National Association of Orthopaedic Nurses (NAON) were sent surveys. Forty-two (21%) NAON members returned completed surveys.FindingsThe activities with the highest Mean Activity Index were primarily those involving pain assessment and pharmacologic management and those relatively specific to orthopaedic patients, such as positioning to increase comfort and premedicating with analgesics before activity.ConclusionOrthopaedic nurses were similar to other nursing specialty groups for assessment and pharmacologic management of pain but placed more emphasis on activity management than other groups.ImplicationsThe results of the study will be used to refine standards of care, develop nursing education curricula, develop research priorities, and develop a blueprint for a certification examination.
