In addition to an individual's academic or educational preparation for a specific job or vocation, the importance of physical readiness has been recognized in the past decade. Physical readiness is particularly needed for individuals injured on the job. Work conditioning and work hardening programs have been developed by the health care community in response to this need. An understanding of the characteristics basic to work conditioning and hardening includes awareness of programmatic and philosophical differences between work programs, knowledge of the interdisciplinary team approach, the powerful rehabilitative tool of communication used by work programs particularly for goal formation, and awareness of each team member's role.

摘要:
This article describes the development and impact of a hospital-based education program for patients undergoing knee or hip replacement surgery. The program consists of three components: a preoperative 1-hour education session conducted by a case manager providing information on what to expect from the procedure; a discussion by previous patients addressing unnecessary fears; and elective membership after discharge in a hospital-based support group providing patients with information on how to live with arthritis and with new joints. The impact of the program on patient outcomes was investigated through a 5-year retrospective study. Data were collected from patients to assess their perceptions of the program and of the outcome of their surgery. Results of the study indicated favorable impact on both patient care outcomes.

Urinary incontinence is most frequently associated with the elderly, particularly women, and is often thought of as inevitable. However, orthopaedic nurses encounter incontinence as a response to alterations in orthopaedic health. This article reviews the types of urinary incontinence, with emphasis on those which may most directly result from orthopaedic problems. It will also help the nurse differentiate between types of incontinence in a way that will clarify appropriate interventions.

Despite potentially having had as part of their educational programs an ethics course or specific classes addressing ethical issues, many nurses still feel ill-equipped when ethical dilemmas occur. This article discusses the Code for Nurses as a resource that nurses can use for determining the appropriate action(s) when an ethical dilemma arises.

Nurses are frequently called upon to develop and evaluate printed education materials (PEMs) in their role as patient educators. This article describes the use of the Evaluating Printed Education Materials (EPEM) model as a prescriptive guide and quality standard for developing new PEMs or critiquing existing ones. Outlined in the five phases of the model are a series of nursing, learning, and instructional design principles that are intended to increase the relevance, readability, and comprehensibility of PEMs for the patients and families who use them. The patient-centered focus of the model makes it applicable across nursing specialty areas and care settings. Examples of how the model can be used in the care of orthopaedic patients are presented.

Recent reports indicate that nurses spend less time with their patients today than in the past (Hendrickson, Doddato, & Kovner, 1990; Misener, Frelin, & Twist, I987 & Quist, I992). This may lead to high frustration levels, low morale, and compromised patient care. Unlicensed assistive staff (patient care assistants and unit assistants), when used appropriately, can provide essential support and make a significant contribution to the delivery of efficient and effective patient care. Key issues, when using these staff members, are role clarity and appropriate delegation. Increased health care technology, the proliferation of nursing and medical knowledge, and new health care delivery models have impacted upon the way nurses and unlicensed assistive personnel work together. The way in which these staff members are organized and work together not only will affect the quality of nursing care that is delivered but outcomes of care as well.
