摘要:
Reflex sympathetic dystrophy, also known as complex regional pain syndrome type I, is a multisymptom syndrome usually affecting one or more extremities. It is inadequately understood and, therefore, often frustrating to treat. This article presents a case study of a 23-year career nurse who developed reflex sympathetic dystrophy of the left knee. It also reviews the rationale for reflex sympathetic dystrophy, treatment, and life-care planning for a patient with reflex sympathetic dystrophy.

PurposeThe purpose of our study was to examine the effectiveness of general health-promotion teaching for patients in the waiting room of a clinic, using focused videotape instruction.DesignAn experimental design was used.SampleSubjects were patients (N= 215) in the waiting rooms of clinics in a university medical center in the Midwest.MethodPatients were randomly assigned to two groups: focused videotape instruction in the clinic (n= 106) and control (no instruction in the clinic waiting area) (n= 109). The outcome measures included patient learning about a health education topic and patient satisfaction with overall care, explanations by the provider, and education received during the clinic visit.FindingsThere was a significant gain in knowledge for patients who viewed the videotape in the waiting room (t= 5.43,df= 213,p< .0001), and they were more satisfied with their education compared with the control group (t= 4.73,df= 213,p< .0001).ImplicationsThis study supports focused video instruction as an effective and efficient teaching intervention for disseminating health information in the waiting area.

摘要:
Since the days of Hippocrates, scripts have included descriptions of infants who were unable to move their arms. However, it was not until the mid-1700s that an obstetric cause for the paralysis was considered. In 1872, the termobstetrical brachial plexus palsywas coined when a correlation was made between excessive traction on the brachial plexus during delivery and the clinical finding of arm paralysis. Surgical intervention became the norm in the beginning of the 19th century and continued until 1930. Poor outcomes and spontaneous resolution of obstetrical brachial plexus palsy prompted a 40-year span of conservative treatment. By the late 1960s, advances in technology and microsurgical techniques revived interest in surgical intervention in the management of obstetrical brachial plexus palsy. This article focuses on obstetrically caused brachial plexus injury, including risk factors, clinical presentation, and treatment options and outcomes. An understanding of current medical practices and their outcomes also provides a basis on which to develop sound support strategies to help parents who face this dilemma.

Nerve entrapment conditions and peripheral neuropathies are sometimes related to injurious events, toxicosis, and repetitive microtrauma to soft tissue and nerve structures. An interesting theory concerning peripheral entrapment neuropathy has been discussed in the literature. This theory is called the double crush syndrome. This article briefly discusses the biologic/metabolic, structural, toxicogenic, and vasculopathic rationale for the pathogenesis of double crush syndrome.

According to the Centers for Disease Control (2002), each year nearly 150,000 Americans die from injuries, making injury the leading cause of death among children and young adults and a significant problem for Americans of all ages. Nonfatal injuries account for approximately 114 million physician office visits and 25% of all emergency department examinations. It is estimated that one in four Americans will suffer a potentially preventable injury serious enough to require medical attention (Shienfield Gorin & Arnold,1998).As injury-prevention advocates, we must address this public health problem with a multidimensional approach that includes the three Es: education, engineering, and enforcement. Professionals from many disciplines, including nurses, physicians, health educators, school teachers, social workers, police officers, firefighters, emergency medical technicians, and coaches, must have accurate information to educate. Education in injury prevention must be a collaborative approach. These injury-prevention leaders must stay informed and knowledgeable of this ever-changing science. This article highlights common injuries, preventive strategies, and resources for injury-prevention information. This information will assist the healthcare provider in finding accurate sources of information concerning injury prevention (Shienfield Gorin & Arnold, 1998).
