&NA;The science of wound care has evolved over the past 20 years. Trends in health care including clinical practice guidelines, evidence‐based practice, and disease‐state management have strengthened the practice of wound management. This article discusses recent strides in this field that may be applicable in orthopaedic nursing. Therapeutic tools discussed include holistic patient assessment, patient health enhancement protocols, therapies and modalities for optimization of the wound bed, and new and developing topical treatments for chronic and acute wounds.

&NA;Effective nursing care of the adolescent undergoing spinal fusion requires attentive nursing interventions in response to a complex situation. The nurse's knowledge of psychological dynamics of adolescence, together with an empathetic attitude and ongoing teaching, helps the adolescent maintain an integrated ego throughout the dependent, postoperative stage. The adolescent's struggle for independence is exacerbated during this time of heightened body image and self‐identity. Help from the “patient unit” (parents, caregivers, and peers) should be kept in focus by the nurse, as empowering this influential unit to take a proactive stance toward patient care aids the adolescent's emotional and physical healing. Balancing medical, nursing, psychological, and spiritual support is the keynote of postoperative care, which must be based on knowledge of adolescent psychological development as well as medical and nursing scientific principles.

&NA;Forefoot pain is common in middle‐aged persons and in the athlete. Increased pressure on the forefoot from too‐narrow, high‐heeled, or ill‐fitting shoes places the foot in abnormal postures and causes pain to foot soft‐tissue and osseous structures. Appreciation of the pathogenesis of interdigital perineural fibroma (also known as Morton's neuroma) will provide a better understanding of this foot malady and will assist patients and health care providers in maintaining better foot health.

&NA;This companion article to interdigital perineural fibroma (a.k.a. Morton's neuroma [MN]) discusses the anatomy and physiology of the nerves in the forefoot, clinical presentation, diagnosis, and treatment of this painful forefoot condition. Although no empirical causal data exists to associate MN with Morton's syndrome or Morton's toe, a condition has been described in the literature whereby particular foot architecture has been linked to anomalies in the first and second metatarsals. This alteration in forefoot structure creates changes in foot biomechanics, precipitates foot pain, and alters foot functioning. To juxtapose this article with the possibility of Morton's syndrome, a family vignette referred to as “Jane's Genus” will be provided as an example of the prevalence of Morton's toe in one family.

Purpose:To compare the outcomes of preoperative education provided in a noninteractive versus an interactive DVD program.Design: Experimental.Sample: Convenience sample of 58 elective joint replacement patients.Method:Subjects were randomly assigned to the video or DVD group. Measurements included posteducation test of knowledge, patient satisfaction questionnaire, and postdischarge collection of data on physical therapy participation, complications, pain behaviors, and length of stay.Findings: Participants in the DVD group had statistically higher knowledge scores and significantly more physical therapy visits. No statistical significant differences were noted in pain behaviors, rate of complications, patient satisfaction, or length of stay.Implications:As our society continues to develop computer technology, opportunities for health care organizations to partner with businesses in developing new‐age patient education programs will increase. Further study is needed to establish the efficacy of DVD instruction (versus other educational approaches) on postoperative clinical outcomes. Testing on larger samples and with other surgical conditions would be valuable.
