Osteogenic sarcoma (OS) is the most common solid tumor of adolescence. Because of the high risk of recurrence and metastatic spread, chemotherapy and surgical interventions are aggressive and potentially dangerous. This article addresses normal growth and development of the adolescent, diagnosis and treatment, and the nursing implications of caring for the adolescent with OS.

Historically, treatment for all forms of cancer has been radical surgical excision, possibly followed by radiation and hope, and in the last event, by chemotherapy. Early referral to a comprehensive cancer treatment center provides the patient with the best hope of cure. This article will provide information on the therapy of the malignant primary bone tumors most frequently seen at The University of Texas, M.D. Anderson Hospital and Tumor Institute at Houston. Emphasis is on the multimodal treatment of the three tumors on which chemotherapy is having its greatest impact: osteosarcoma, Ewing 's sarcoma, and malignant fibrous histiocytoma (MFH) of bone.

摘要:
The majority of patients undergoing limb-salvage procedures for the treatment of musculoskeletal tumors have demonstrated good to excellent function at a 1-year follow-up study at the Cleveland Clinic Foundation. Sixty-two patients having surgery for the treatment of sarcomas were evaluated at 2-month intervals and rated according to symptoms, range of motion, strength, functional abilities in relationship to the activities of daily living, and ambulation and transfer techniques. Potential alterations in mobility and function are directly related to the extent of surgical resection, state of health before surgery, and psychologic status before and after surgery. Nursing interventions can help enhance the patient's ultimate functional result. However, despite the potentially good to excellent results in a majority of patients, complications can result and impair function. The maximum result is normally achieved at about 1 year after surgery.

Adjusting to an amputation is even more difficult for adolescents since they are in a period of transition with overwhelming developmental challenges. Nurses can have the greatest impact when they understand the period of adolescence as well as the four-phase process of adjustment to limb amputation: impact, retreat, acknowledgment, and reconstruction.

A former osteogenic sarcoma patient, now a cancer nurse, recalls the wide range of emotional needs she experienced as a cancer patient in a large teaching hospital. Chemotherapy treatments, combined with numerous surgical attempts to save her leg, created complications that later necessitated an above-knee amputation to save her life. Convinced from both the patient's and nurse's perspectives that empathetic nurses have a crucial role in helping osteogenic sarcoma patients effectively cope with the emotional stress caused by various cancer treatments, the author recounts some of her experiences in an effort to make nurses aware of patients' holistic needs, as well as ways to provide psychologic support.

Caring for a patient who undergoes an amputation for osteogenic sarcoma (OS) can be both challenging and satisfying. This article, in case study format, focuses on the experience of a 24-year-old male patient who had a right arm disarticulation (amputation through a joint). The nursing process and team approach used to care for this patient and his family are discussed.

This article reviews the causes of cancer pain, nursing assessment, and intervention. Bone pain and phantom limb pain, which are cancer-related pains of particular interest to orthopaedic nurses, are discussed in detail. Examples and case studies are used to illustrate nursing care of patients with bone pain or phantom limb pain.
