摘要:
Although postoperative blood salvage is used routinely in orthopaedic surgical procedures, little data exists to evaluate this practice. The purpose of this study was to evaluate the effectiveness of the Solcotrans®autotransfusion system following elective total joint arthroplasty. This study compared two groups of patients: 59 patients who used the Solcotrans Orthopaedic Drainage/Reinfusion System and 56 patients who used a standard drainage system. Based on the results of the study, the researchers questioned the clinical efficacy and cost-effectiveness of a postoperative wound drainage/reinfusion system in patients after elective total joint arthroplasty.

摘要:
The purpose of this study was to compare the costs and outcomes of the postoperative autoreinfusion device, the Constavac™, with the standard suction device, the Hemovac™, in which blood drainage is discarded, in patients undergoing total joint arthroplasty of the knee or hip. At the completion of the surgical procedure, a total of 91 subjects were randomly assigned to one of the two device groups. The findings of this study, which included a limited homogeneous sample, do not support the use of the Constavac™device in patients undergoing total joint arthroplasty.

摘要:
The leading cause for low back pain and sciatica is a herniated intervertebral disc, which has often been removed by laminectomy or chemonucleolysis. A less invasive method is percutaneous lumbar discectomy, surgical aspiration of the central nuclear material. With proper patient selection and surgical technique, percutaneous lumbar discectomy achieves results similar to those of chemonucleolysis.

Deep venous thrombosis (DVT) and its complications, pulmonary embolism (PE), and postphlebitic syndrome (PPS) affect millions of Americans. The risk is especially high in surgical patients in general, and orthopaedic patients in particular. Virchow's triad describes the three factors that affect coagulation within a blood vessel: stasis, vessel injury, and coagulation factors. Risk factors, which relate to these factors, can predict which patients will be most likely to develop DVT. Prophylactic regimens should be tailored to patient risk. Nursing care is the key to implementation, maintenance, and compliance with any prophylactic program and its ultimate success.

Delegation is a learned skill that is critical for the orthopaedic staff nurse today. The evolution of assistive personnel and the health care delivery system is introduced, along with who unlicensed assistive personnel are and distinctions between delegation and assignment. The scope of nursing practice is discussed in relation to nurse practice acts, standards, position statements, and evaluation of practice. Several models, including the nursing process, present criteria for delegation. Barriers to delegation involve characteristics of the staff nurse, issues of supervision, and reverse delegation. Benefits are noted that will encourage the orthopaedic nurse to delegate legally and effectively in an effort to work efficiently and to empower colleagues.
