Osteoporosis is attracting a great deal attention. This condition strikes 15 to 20 million Americans, mostly women. The role of calcium and estrogen in the prevention and treatment of postmenopausal osteoporosis is only beginning to understood. This article describes the pharmacodynamics of calcium and estrogen and recommendations for their use. Calcium and estrogen are only two of the factors believed to play a part in the development of this condition. Future research may indicate that a combination of preventive and therapeutic modalities be prescribed.

Osteoporosis is an age-related disorder characterized by loss of bone mass resulting in increased risk for fractures, particularly in the lower thoracic and lumbar vertebrae. Clinically this results in acute or chronic back pain. Women at risk are postmenopausal, white, and thin. Acute treatment emphasizes pain control and prevention of complications. Prevention of further bone loss and subsequent fractures is the goal of long-term treatment. Nursing care centers on pain relief, gradual increase in activity, and education. This article discusses osteoporosis and its relation to spine fractures, its clinical features, risk factors, treatment, and nursing care.

Glucocorticoid excess, in any form, is deleterious to bone metabolism, and leads to a rapid and profound decrease in bone mass. The most severe effects occur in the axial skeleton where the greatest concentration of trabecular bone is found. When the bone density falls below the level at which the body can support its own weight, pathologic fractures may result. These occur most commonly in the vertebral bodies, ribs, hips, proximal humeri, and distal radii. An understanding of the pathophysiology of glucocorticoid osteopenia is essential for the proper management of this condition.

摘要:
Renal osteodystrophy is present in 50–80% of patients with chronic renal failure. The bone pain and accompanying muscular weakness and limitations complicates an already complicated regimen. Prevention of osteodystrophy through maintenance of normal calcium and phosphorus levels is the key to management. Nurses play a major role in supporting and facilitating compliance with these prescribed regimens. Orthopaedically, the fractures of osteodystrophy are managed in the same way as the patient with osteoporosis.
