Use of nursing diagnoses allows nurses to name and classify the judgments nurses apply to analysis of a patient's assessment data. Such classification systems are an important part of the proposed Nursing Minimum Data Set (NMDS) (see sidebar on next page). This article describes the findings of NAON's survey of members' (N = 1500) use of nursing diagnoses in the care of patients undergoing surgery and major invasive procedures. The response rate for this survey was 37.5% (n = 563) and found to be representative of NAON's general membership. Seven nursing diagnoses were reported as primarily used during the care of surgical patients. Six nursing diagnoses were reported to be applied in the care of patients undergoing significant invasive procedures. Findings of this study may be used to contribute orthopaedic nurses' unique perspective to the formation of the Nursing Minimum Data Set (NMDS).

Epicondylitis can be a debilitating condition that restricts activity and causes pain. Severe cases can require surgical repair. Early and thorough assessment and instituting treatment expeditiously will help prevent lengthy rehabilitation. It is well documented that sports requiring repetitive wrist movements or tension overloading of wrist extensors, such as racquet sports and bowling, are associated with the development of epicondylitis. In this case study, the sport of karate precipitated this condition in a 38-year-old instructor. The significance of karate as a possible risk factor in epicondylitis is important given the proliferation of karate schools, especially those for children.

Cauda equina syndrome (CES) is a presentation of signs and symptoms that indicate nerve compression involving one or more of the nerve roots in the lumbar spine. The Clinical Nurse Specialist working in a general orthopaedic practice must become familiar with the presentation of CES to appropriately assess the patient and alert the physician. Prompt and accurate care of the patient is critical when cauda equina syndrome is suspected to minimize permanent nerve damage. This article describes the syndrome, its signs and symptoms, basic triage techniques, plan of care, and recovery for the patient with cauda equina syndrome.

Change initiatives (strategic plans) often fail, not because the idea was wrong, but because the change process failed to consider the human side of change. Achieving desired change outcomes requires an accentuation of transition management efforts or a focus on people. This article proposes 12 steps to managing transition and preparing “change-ready” employees and highlights strategies for both staff and managers to consider in the process.

Surgical positioning is a very important aspect of caring for the patient intraoperatively. All surgical team members must work together to safely move and secure the patient to adequately expose the surgical site. The type of surgical position, anesthesia used and the patient's health status continue to be factors which contribute to optimizing the patient's outcome and preventing complications. It is imperative that perioperative nurses understand and are competent in utilizing principles of anatomy and physiology, knowledge of effects of medications and anesthetics, critical assessment skills and appropriate positioning techniques and equipment used during orthopaedic surgical procedures to render quality intraoperative care.

The implementation of clinical pathways in home care, along with the use of outcome tools and the development of benchmark physical therapy guidelines, is discussed in this article. Three tools were developed by an interdisciplinary team led by an orthopaedic surgeon and the clinical nurse specialist (CNS) in cooperation with several members of the home care staff.
