摘要:
Cast syndrome, clinically known as superior mesenteric artery syndrome (SMAS), is gastric dilatation with partial or complete obstruction of the duodenum. Although rare, it is most frequently seen in orthopaedic patients who have had spinal surgery or who are in hip spica or body casts. Obstruction occurs when there is compression of the duodenum between the superior mesenteric artery anteriorly and the aorta and spinal column posteriorly. Obstruction can occur within days of surgery or casting or may not develop for several weeks. Treatment for SMAS varies from conservative nonoperative to operative procedures. Complications can be severe if symptoms are not quickly recognized and treatment instituted in a timely manner.

To remain competent in practice, as the demands of that practice change, and to take advantage of new roles and opportunities in nursing, three behaviors are required on the part of the nurse: ability to evaluate one's own knowledge and skills for practice, an awareness of resources available for development of new competencies for practice, and a willingness to engage in this self-assessment. This article explores these behaviors essential for the professional development of the nurse and includes specific strategies for assessing one's own learning needs and meeting them.

Falls often lead to the fracture of bones in the elderly population. Fall incidence is increasing with the concomitant rise in the elderly population. Recently, some nontraditional methods of fall prevention have been explored. This article provides some background information about the ancient practice of Tai Chi as well as a brief review of the current literature exploring the effectiveness of Tai Chi in health promotion and fall prevention.

A mobility health assessment is necessary for clients with suspected mobility impairment or after sustaining trauma or injury to identify the need for further referrals and assistive devices. This article covers the nurse's role and the three components of a thorough mobility assessment. The essential elements of a complete history, review of relevant diagnostic tests and physical assessment are discussed. Guidelines for range of motion of all joints are presented and illustrated.

摘要:
In 1992, a preoperative education program was developed for total joint replacement patients in a small community hospital. The goals of the program were to increase educational opportunities for the joint replacement patients, prepare patients for hospitalization, plan for discharge needs, and increase efficiency of the orthopaedic program. Since 1992, approximately 600 patients have attended the education program. Outcomes have included positive responses from patients regarding their preparedness for surgery, increased participation in their plan of care, coordinated discharge planning, decreased length of stay, and progression across the continuum of care. A multidisciplinary approach to preparing patients for surgery allows for a comprehensive and efficient education program. Marketing of successful programs can enhance an institution's competitive advantage and help ensure the hospital's viability in the current health care arena.

PurposeTo examine whether patients who received an empowerment model of education for preoperative orthopaedic teaching had improved outcomes compared to patients who received the traditional education.DesignAn experimental (empowerment teaching method) group vs. comparison (traditional teaching method) group posttest design.SampleSeventy-four patients undergoing elective orthopaedic surgery.MethodsFollowing the preoperative teaching session, patients in both groups completed a questionnaire designed to measure their perceptions of the teaching (empowerment) and self-efficacy (belief in their ability to carry out perioperative tasks). A chart audit and phone interview was done after discharge to assess length of stay, pain management, complications, and patient perceptions of the ability to complete perioperative tasks.FindingsPatients in the empowerment group felt the educational approach was more empowering and had significantly higher self-efficacy scores than those in the traditional teaching group. There was much less variation in empowerment and self-efficacy scores in the empowerment group. The empowerment group reported feeling greater confidence in performing perioperative tasks. There were no differences in length of stay, complications or pain control.ConclusionUse of an empowerment teaching approach enabled patients to become more confident in their ability to carry out perioperative tasks and become a more integral part of the preoperative teaching process.Implications for Nursing ResearchThe theoretical model will be used to structure other educational programs and guide research. More sensitive measures of complications and pain control should be considered for future studies.

摘要:
Why is someone other than a competent adult patient considered to be the person to make important treatment decisions? Since one's values influence decision making, who other than the patient should make these significant decisions? The author argues that competent adult patients should make treatment decisions because of the moral nature of these decisions. Recommendations for facilitating the patient's active involvement include increasing one's understanding of values, autonomy, and decision making; identifying personal values; creating a supportive environment; educating patients about their role in decision making; and advocating for patient involvement in treatment decisions.
