Since 1970 there has been a dramatic increase in the number of total hip arthroplasties done in this country. Many patients have a new lease on life following this operative procedure. There has been a tremendous change in the operative procedure itself, the prostheses, and the preoperative and postoperative care of patients over the past 25 years. This article addresses the present day procedure as well as current methods and concerns for nurses in the care of the patient with total hip arthroplasty.

Health care consumers and providers are looking for economical, noninvasive, nonpharmacologic alternatives to traditional medical care. Holistic therapeutic nursing interventions expand the nurse's repertoire of therapies to promote health and augment or replace standard pharmacologic interventions. Standards of Clinical Nursing Practice require nurses to maintain current knowledge, use research findings, and contribute to cost-effective use of resources. Use of selected holistic interventions such as music, humor, and reminiscence enables nurses in any setting to maintain professional standards and offer additional alternative interventions to their specific patient population.

Neurofibromatosis is a genetically transmitted, multisystemic disorder characterized by abnormalities of the skin, nervous tissue, and bone. Many of the serious problems associated with this disease are orthopaedic in nature. The treatment and plan of care for a child with neurofibromatosis must be comprehensive and consistent. Nursing care should address the developmental, physical, and emotional needs of the child, as well as the psychologic concerns and educational needs of the child and parents.

Neurofibromatosis (Von Recklinghausen disease) is a genetically transmitted, multisystemic disorder characterized by abnormalities of the skin, peripheral nerves, bone, and soft tissue. Symptoms appear early in life and vary with the type and severity of the disease. Care of patients with neurofibromatosis (NF) presents a complex challenge for nursing, the multidisciplinary team, and the family. This article profiles the progression of neurofibromatosis in one girl from birth through adolescence. The author emphasizes that while health care workers ably provide expertise and assistance, the ultimate responsiblity for decision making rests with the child and her family.

The precise cause of rheumatoid arthritis (RA) is, as yet, unknown. But with more sophisticated techniques in the fields of immunogenetics and molecular biology, there is increasing knowledge of the pathophysiology of the rheumatoid joint. Pathophysiologic knowledge should be part of the orthopaedic nurse's repertoire when dealing with the “whole” patient; therefore orthopaedic nurses who care for patients with rheumatoid disease should understand certain pathophysiologic concepts. This article reviews the pathophysiology of the rheumatoid joint and describes the changes that take place in the joint with specific reference to the cells of the immune system, the synovium, and articular cartilage.

Rheumatoid arthritis affects joints and other systems in the body. Dryness of the eyes and mucous membranes are referred to as Sjogren's Syndrome. The heart is usually spared but the lungs can be affected by pleurisy, scarring, and the formation of nodules in the lungs. The nervous system may be involved by compression neuropathy (e.g. carpal tunnel syndrome), peripheral neuropathy or occasionally from cervical cord compression. The rare complication of vasculitis (inflammation of blood vessels) may be devastating because of gangrene or severe internal organ damage. Some of the effects of rheumatoid arthritis may be “invisible” such as anemia or abnormalities in the white blood cell count. Osteoporosis is usually asymptomatic until a fracture occurs.

Our society supports the right of its members to be self-determining and to make decisions based on their personal values and beliefs. However, what happens when a person lacks the capacity to make decisions? The author identifies criteria for determining decision-making capacity and discusses the surrogate decision maker, the best interests standard, and the substituted judgment standard. Nursing implications focusing on treating the patient with dignity and respect and protecting the patient's rights are discussed.
