摘要:
The rate of repetitive strain injuries (RSI) in the workplace is rising at an alarming rate. People using computers and working at keyboards for long periods of time are particularly at risk. Prevention of orthopaedic injuries can make the work environment less stressful and more productive as well as help to avoid needless human suffering and costly medical and legal bills. Knowledge of the causes of RSI and preventive measures to reduce orthopaedic hazards of the computer work environment is important for orthopaedic nurses.

For established cases of CTS, conservative measures such as splinting and medication are the primary treatment with surgery an option when conservative treatment fails.

摘要:
Patients with acetabular fractures unamenable to open reduction and internal fixation are subject to 6-12 weeks of traction and, traditionally, bed rest. Using roller traction allows the patient to be mobilized to a bedside chair and to stand without disrupting the traction. This is accomplished by slightly modifying a standard lower extremity skeletal traction setup. Roller pulleys and stopper clamps are substituted for the usual pulleys. These are attached to extra-long horizontal bars that expand the lateral space that the patient can now use around the bed. Roller traction helps prevent the common complications of prolonged bed rest, particularly deep vein thrombosis, skin breakdown, pulmonary stasis and boredom.

Changes in bowel elimination patterns (BEP) for elders admitted to an acute care hospital is essentially unknown. This exploratory study compared a group of 154 elderly with 149 middle-aged subjects from two northern New England hospitals. Interviews, diaries, questionnaires, chart reviews, the Nagley VAS for Confusion, Katz Index of ADL and MEDISGRPS were used in data collection. Analysis included descriptive statistics, t-tests, and multiple regression. During hospitalization, the BEP of both groups changed significantly, but there was no difference between groups on the degree of change. Multiple regression analysis was used to examine the effects of diet and activity on degree of change in BEP while controlling for effects of gender, severity of illness, functional status, and cognitive status; producing a significant coefficient for only the elderly subjects (R2=.13, p, .01). Therefore, these results support the supposition that acitivity and diet play a greater role in changes in BEP for elderly patients than middleaged patients admitted to an acute care hospital.

Early treatment of developmental dysplasia of the hip in infants is essential for optimal development of the hip joint. Orthopaedists commonly use a period of preliminary traction before hip reduction to stretch the soft tissues and reduce the risk of avascular necrosis. A program developed at the University of Michigan Medical Center allows the infant to receive traction at home instead of the traditional admission to the hospital. This program has been found to be clinically effective as well as cost effective, and less of an inconvenience to the infant and family than the traditional admission. Intensive involvement by the clinical nurse specialist in teaching and follow-up of each patient is essential for success.

Data from an open-ended questionnaire and the comments section of a structured questionnaire were combined and analyzed to determine the challenges faced and alterations in usual daily activities required by mothers caring for a young child at home in a body cast. The sample consisted of 35 mothers who identified themselves as the primary care givers of a child in a body cast. The challenges of caring for a child in a body cast required major adjustments in almost every aspect of the mothers' lives, including household activities, social and community activities, child care activities, personal care activities, occupational activities, and educational activities.

While shoulder dislocations are a common injury, dislocation in the posterior direction is unusual and often missed clinically. A traumatic posterior shoulder dislocation can be caused by a direct blow to the anterior shoulder or if a posteriorly directed force is axially applied through the forward flexed arm. A posterior shoulder dislocation can occur after a violent muscle contraction induced by an electrical shock or grand mal seizure. Posterior shoulder dislocations are commonly associated with posterior glenoid rim fractures and anterior compression fractures of the humeral head. This diagnosis should not be missed if a complete physical and radiographic exam are properly performed.
