This study was a secondary analysis of an existing data set. The purpose was to explore the problem-solving cognitive structure that nurse practitioners (NPs) used in their assessment and management of a patient with acute pneumonia superimposed over chronic bronchitis, as measured by a patient management problem with 296 items. Results of a factor analysis suggested that the activity of patient education was different from assessment and medical management functions. A discriminant analysis was used to explore which variables correlated with high and low performance on the simulation. Laboratory proficiency scores were the first, patient education management the second best discriminating variables. Routes through the simulation were labeled typical, suggesting the usual sequence of history, physical, laboratory tests, and management, and atypical, suggesting variations on this route. The typical group performed significantly better than the atypical group. At the end of each section within the simulations, subjects were asked to list potential problems and possible medical diagnosts. Subjects were able, with a minimal amount of data, to list correct problems and diagnosis and appeared to proceed through the simulation attempting to confirm or deny those hunches. These findings support the concept that there appears to be a nursing focus withing NPs' performance on a clinical simulation.

A multivariate study investigated developmental progress in very low birth weight infants (VLBW ≤ 1,500 grams) during the first year of life. The subjects were 41 VLBW infants (Mbirth weight = 1,203 grams) who were free from congenital anomalies and appropriate for gestational age. Eighteen infants were male, 23 were female. Data were gathered from the infants' charts and during home visits when the subjects were 6 and 12 months corrected gestational age. Birth weight, intraventricular hemorrhage status, number of days supported by mechanical ventilation, number of days in the intensive care nursery, socioeconomic status, and characteristics of the childrearing environment were examined in relation to developmental progress. Data were analyzed using multiple regression analysis and cross-lagged panel analysts. Environmental factors were powerful influences accounting for 67% of the variance in development at 1 year. Cross-lagged panel analysis suggested a causal relationship between the quality of the home environment and developmental achievement at 1 year.

The purpose of this study was to: (a) design and test an instrument to evaluate the incidence and severity of stressors associated with coronary bypass surgery and (b) assess the concordance between patient and nurse perceptions of such stressors. The patient sample consisted of 30 patients recovering without complications from coronary bypass surgery. Patients were white, primarily male and employed, one-third with graduate education, with a mean age of 54. The nurse sample consisted of 18 registered nurses with cardiosurgical nursing care experience. A 30-item stressor scale was developed: 27 items were derived from literature review and 3 from comments of cardiovascular clinical specialists. Stressfulness of each item was rated on a 5-point scale. A pilot study with seven patients provided information for initial scale refinement. Content validity of the scale has both theoretical and empirical support because of the varied sources used to obtain and validate relevant items for the tool. Homogeneity reliability of the scale was supported by high coefficient alphas. Based on severity of stress ratings, two-way analysis of variance indicated that the cardiothoracic nurses generally rated all items as significantly more stressful for coronary bypass patients than did the patients themselves. Comparisons used by the nurses and the patients in assigning stressfulness ratings are discussed.
