SummaryThe purpose of this part of the research project on clinical inference in nursing was to attempt to identify what the message units of various cognitive tasks in nursing are. The effort failed.No single cue was found to convey more than a trivial amount of information to the nurse-subjects. Nor were groups of cues, arranged in various ways, found to be related to the inferences made by the nurse-subjects about SOP. And, finally, it was learned that these nurse-subjects did not consciously discriminate between the usefulness of various cues, nor did their confidence in their decisions vary over cases.The study contained various flaws: 1) the cases were not representative, in a strict sense, of cognitive tasks encountered by nurses; 2) the data were crudely presented in the cases, and were not in a form usually seen by the nurse; 3) the statistical analysis was inadequate; the number of cases (100) was not sufficiently large for each analysis made; and 4) the various cue-groupings employed were, to a large degree, arbitrary. It may well be that, if none of these deficiencies existed, the basic message units of cognitive tasks would have been discovered. It is doubtful, however, that this would have been the case, for the following reasons: 1) for the most part, the defects listed above would not have prevented the appearance of regularities between cues (and/or cue-groupings) and inferences if they were actually there to be discovered; 2) there were extremely wide individual differences in the six nurse-subjects in their inference patterns drawn from the same 100 cases—if basic message units do exist, the nurse-subjects employed here do not know of them, or do not agree on them; and 3) the nurse-subjects did not discriminate among the usefulness of cues. If basic message units are common knowledge, the nurse-subjects did not employ them in a consistent fashion.Thus, the results of the present study are not conclusive. They do indicate, however, that questions concerning the message units involved in cognitive tasks encountered by the nurse can be investigated. The question of what information units are used by the nurse should be answered.

SummaryA total of 103 intravenous infusions were studied to identify factors related to infiltration. Findings suggested that infiltration was related to 1) length of needle—none of the infusions started with a 1 1/2-inch needle infiltrated, whereas 21 out of 83 started with a 1-inch needle infiltrated; 2) activity of the patient; and 3) above average amount of nonprofessional nursing hours. No significant relationship was found between infiltration and site of the infusion (hand or antecubital fossa); method of taping the needle; gauge of needle, vein condition; sex or age of patient; and length or number of infusions.Other factors which might influence infiltration and which also warrant study are: 1) the interrelationship of needle length and needle gauge, 2) bevel length, and 31 interrelationship between upper extremity infusion site and handedness of the patient. The present study has pointed out variables that need to be subjected to controlled research to determine the relationship to infiltration.

SummaryA 6-hour programmed unit on preoperative nursing care of the amputee was developed and tested. The sample consisted of 26 nursing students, 13 in the control group and 13 in the experimental group. Findings indicated that the group on which the programmed instruction was tested tended to do better on the immediate post-tests and to retain the learned material longer. Experiments of larger scope need to be conducted to confirm these findings.

SummaryFactors related to the illness behavior of two graduating classes of nursing students (N = 109) were studied. The relationship of psychiatric referral, college gradepoint average, size of the home community, occupational class of the father, number of siblings, and religious preference were studied by multiple regression techniques as they relate to the frequency of visits for health services, and academic performance in nursing subjects. The only significant predictive variable for visitation rates was a history of psychiatric consultation. The only significant predictor of nursing gradepoint average was the undergraduate gradepoint average. The equation for academic performance in nursing subjects suggests relative underachievement of superior students. The nursing student group had the highest utilization rate of health services and psychiatric consultations of any student group on the medical center campus. There was no evidence that the frequency of visits or referrals to psychiatry were correlated with any particular course work, examination schedule, or stage in the educational program. The implications of these findings are discussed.
