摘要:
Theories of touch as related to nonverbal communication in the area of patient care are examined, and studies of touch as a general communication device and as a specific factor in the medical discipline are reviewed. According to the abstract universal of touch communication, as observed in use in american culture today, five broad areas are developed into a concept of touch. Out of this concept, inferences are formed into a “construct” a theoretical abstraction of observed facts that th act of touch is an integral part of nursing intervention and is to be used judiciously between nurse and patient, health team and patient, and health team and nurse as a fundamental mechanic of communication, a vital means of communicating, a basis for establishing communication, and as an important means of communicating emotions and ideas.

A study of self-actualization and role adaptation was carried out using a sample of baccalaureate degree nurses who were part of an ongoing larger study of collegiate graduate nurses. (Of an original 222 nurses in 37 medical center hospitals in 1967, 195 were tested in 1970 for the study reported; 112 were still in hospital nursing.) Dependent variables were taken from the 12-scale Personal Orientation Inventory (POI) which tests Time Competence/Incompetence, Inner-directed/Other-directed support, and ten subscales considered important for self-actualization: valuing, feeling, self-perception, awareness, and interpersonal sensitivity. Independent variables concerned attitude and behavior roles: professional role conception, bureaucratic role conception, role deprivation, integrative role behavior, job success (high, average, or low), and job status. Six hypotheses were tested. A factor analysis of the POI scales was done to condense and group the dependent variables; the three derived factors were uncorrelated. Using the POI scales of Time Competence (Tc) and Inner-directedness (1) as shorthand indicators of self-actualization, no significant main effects were obtained to discriminate between the independent variables. Interactive effects between nurses holding varying degrees of strength on independent variables were obtained; the effect of varying strengths of adherence to the bureaucratic work system and to the professional code were separately and jointly interactive with job success when the Time Competence aspect of self-actualization was examined and jointly interactive when the Inner-directed scale was used.

In a study to describe technical (associate degree) and professional (bacalaureate) nursing as practiced in clinical settings by staff nurses and to determine whether differences are noted by directors of nursing and head nurses, 24 associate degree and 24 baccalaureate staff nurses were observed and interviewed and 12 directors of nursing and 22 head nurses were interviewed in 12 hospitals in the San Francisco Bay area in which graduates of both programs are employed. Differences between technical and professional practice were identified in areas of problem-solving and decision-making, in the scope of practice, and in attitudes toward practice. Actions and attitudes of associate degree graduates were found to be consistet with technical nursing practice. Actions of these nurses had predictable outcomes; the nursing problems and interventions were physiological and physical. Baccalaureate graduates were found to practice professional nursing, to consider patients' psychological and social needs. Bases for practice of six of the 24 baccalaureate graduates extended beyond standardized approaches; the nurses were self-directed and willing to take a chance. Interviews with directors of nursing supported differences in practice, but head nurses, while reporting differences, cited examples not entirely supportive of their original statements.

Concerned with the effect a learning experience exerts on the nurse, thereby influencing her beliefs and attitudes, the purpose of this study was to investigate the effect of a rehabilitation nursing workshop on registered nurses' attitudes toward their roles in the rehabilitative nursing care of patients with long-term illness. The instrument used to determine the nurses' attitudes consisted of a captioned, line drawing picture packet. Variable selected for study through this projective method included the role of the nurse in self-help activities, in guidance and vocational training, in giving psychological-emotional support, in fostering functional independence, in promoting range of joint motion, in care of the patient with long-term illness, in promoting the therapeutic program, in promoting proper positioning and correct body alignment, and in promoting the team concept. Sixty registered nurses who participated in four-week rehabilitation nursing workshops were tested before and after completion of the workshop. Responses were rated on a five-point positive-negative continuum and pre- and postworkshop scores were compared. Statistical analysis revealed a significant positive change in attitude in all areas with the exception of the role of the nurse in providing psychological-emotional support. When scores were examined with respect to age, educational background, and experience, age was the only variable found to be related to change.

Findings of studies that registered nurses are low in qualities traditionally associated with therapeutic effectiveness in counseling—empathy, nonpossessive warmth, and genuineness—are reported. Investigations which used selected variables from the Edwards Personal Preference Schedule and from other character tests to measure attitudes of secondary school and college students in health fields and in general; nursing students in diploma, associate degree, and baccalaureate programs; and nurses in a variety of settings are compared. Nurses, the review of the literature reveals, are becoming aware of their limitations in therapeutic effectiveness and are attempting to overcome them.

Graduates of five associate degree nursing programs were studied to examine the value of 11 prediction equations in determining 1) whether entering students would eventually take the State Board Examination (SBE), 2) entering students' predicted scores on the five SBE tests for admission purposes, and 3) entering students' predicted scores on the five SBE tests for remediation purposes. Two prediction batteries were used. The first battery included 18 variables which would be available prior to admission to nursing school; the second, those 18 variables plus nursing school semester grade averages and grade averages squared. Students who entered nursing school but did not eventually take the SBE were found to be significantly different from students who did take the SBE. Prediction of the SBE scores was improved by including current achievement information in the prediction battery. Examples of how to use the prediction equations which include preadmission and current achievement information are provided.

摘要:
When examined at a one-day institute on their attitudes toward innovative changes in nursing practice, specifically in intensive coronary care units, practicing nurses and nursing students responded differently according to their job status. The 118 subjects were classified into five hierarchical gourps: nursing directors, instructors, head nurses, staff nurses, nursing students. Higher ranking nurses tended to believe nurses expected negative results from technologic change, while staff nurses and nursing students, on the other hand, considered nurses were receptive to change.
