摘要:
In an attempt to apply the scientific method to quality assessment, a methodological chart is presented which clarifies the meaning and use of measurement and its relationship to decisions about appropriateness of quality assessment procedures. Six quality assessment research issues are proposed and discussed. It is suggested that quality assessors and researchers work together as partners to strengthen methods currently, used to assess the quality of care in day-to-day quality assurance programs.

The importance of differentiating between evaluation (an overview of nursing care) and research (a biopsy of nursing care) is stressed. Evaluations are designed to influence nursing care; research is not. In evaluations, feedback is ongoing and dynamic; in research, variables are controlled for change. Magnitude of occurrence and amenability should determine priority of problem selection.

Because much current health science information is irrelevant to immediate decision needs of quality assurance and care, because much of what is relevant may be inaccessible, and because much of that which is relevant and accessible may be invalid or scientifically unsupportable, health care personnel should work toward resolution of this problem on a national level as well as on an individual level. This article presents evidence supporting the above premises together with data from illustrative publications and a suggested strategy to help cope with the general problem of information management in quality assurance.

From a researcher's point of view, criteria used in developing measures of nursing care should achieve certain standards: They should be based on a conceptual framework that fits the study. The evaluator considers criteria from theoretical and practical viewpoints: The criteria should reflect a conceptual view of nursing care consistent with the ' user's view. Subsets of criteria should be not only statistically sound, but also practical and meaningful. Measures should be adequately unobtrusive in use. Cost-effectiveness, credibility of the criteria with users, and ease of adaptation to change in practice should also be considered.

Simply more research will not answer the problem of evaluation of patient outcomes. Research in nursing related to clinical phenomena lacks a specific focus and sustained pursuit of subject matter. What is required is unifying theory, a recognition that the existence of ambiguity is essential to the creation of science, an open mind, and the ability to criticize oneself objectively.
