The difference in stroke causes between younger and elderly stroke patients may largely depend on differences in the cause of cortical ischaemic stroke, the pathogenesis in lacunar ischaemic stroke being more homogeneous. We therefore compared the vascular risk factor profile and stroke causes between 60 ischaemic stroke patients of 50 years or less and 756 patients older than 50 years, in relation to these two ischaemic stroke subtypes. The stroke causes were classified as small-vessel occlusion, large-vessel atherosclerosis, cardioembolism and as rare definite causes (such as dissection, vasculitis, and coagulation disorders). The rare definite stroke causes were significantly more frequent among young patients (15 vs. 3%, odds ratio 6.2; 95% confidence interval, CI, 2.8–13.6). All these young patients had a cortical stroke. The whole group of lacunar stroke patients (young and old) had significantly less frequently rare definite stroke causes than the whole group of cortical stroke patients (1 vs. 5%; odds ratio 0.22; 95% CI 0.07–0.71). Young patients have more varied stroke causes than older patients, but this is largely confined to cortical stroke patients. The accuracy of a simple isotope method of assessing middle cerebral arterial occlusion was compared with that of transcranial Doppler ultrasound and with a careful clinical examination and CT brain scan to predict the probable pattern of cerebral arterial occlusion in 85 acute stroke patients. The isotope method detected likely middle cerebral artery main stem or large branch occlusion with equal accuracy to transcranial Doppler ultrasound. This simple isotope method is a practical way of assessing cerebral arterial occlusion, is inexpensive, non-operator-dependent, faster than transcranial Doppler ultrasound and uses technology which is already widely available. It may be useful for research in acute stroke at present and in future may help guide treatment of acute ischaemic stroke.

Internal carotid artery (ICA) hypoplasia is a rare congenital anomaly that is sometimes difficult to distinguish from acquired diseases associated with narrowing of the ICA. As the carotid canal develops in association with the ICA, a hypoplastic carotid artery should result in a hypoplastic carotid canal. We used skull base CT scans to measure the size of carotid canals in 4 patients with ICA hypoplasia (bilateral in 1) and in 30 consecutive controls. The diameter of the carotid canal was significantly smaller in the patients with carotid hypoplasia (2.4–3.0 mm, on the involved side) than in the controls (mean = 6.4 ± 0.8 mm, range = 5.2–9.8 mm) with no overlapping value (p < 0.001). The ratio of the largest to the smallest carotid diameter was greater than 2 in patients with unilateral hypoplasia, while it never exceeded 1.2 in controls. Ataxic hemiparesis is a well-recognized manifestation of lacunar infarction. We report a patient in whom this syndrome was caused by a 4-cm3 parasagittal haematoma. The finding that ataxic hemiparesis may be caused by a primary intracerebral haematoma has implications for treatment and warrants early imaging of these patients.
