To measure the incidence and outcome of stroke in Estonia a community-based study of first-ever stroke was carried out in Tartu from 1991 through 1993. A total of 829 first-ever stroke cases were registered during the 3-year period. Most stroke patients were prospectively identified from the Department of Neurology and Neurosurgery of the University Hospital, other sources where death certificates, autopsy protocols, outpatient and hospital records. The hemorrhagic stroke subtype constituted 15% of all cases. Although pathological verification was available in 32% of cases (autopsy or CT/MRI), the incidence and 30 days'' case fatality rates for the hemorrhagic subtype were similar to those of other studies, while the rates for the ischemic subtype were higher than those reported by many authors. The total incidence rate (2.50 per 1,000), the age-specific rates for the patients < 65 and the overall case fatality rate at 30 days were higher than those found by most study centers. Possibly, a study of stroke risk factors would explain the high burden of stroke in Estonia.

In 1992, there were 48,038 stroke patients hospitalized in Austria, which is equivalent to a stroke incidence of 609/100,000 (781/100,000 standardized for age, 35 years and older). The average number of hospitalizations per patient was 1.1 times per year; the mean length of stay in the hospital was 24.7 days per patient and 22.5 days per hospitalization. This accounts for a total of 1.2 million hospital days, worth US$ 344 million. Based on the current level of incidence and medical care, the costs related to stroke will rise by 25% until the year 2010, as the proportion of elderly people continues to grow. Data on past trends and future projections of stroke in Austria and other countries form the necessary basis for planning prevention and control of stroke.

One hundred stroke patients recovered from the acute phase and undergoing geriatric hospital rehabilitation were included in a study aimed at finding factors predicting discharge disposition. Background characteristics, functional status at admission and at discharge, complications during hospital stay, length of stay, and discharge disposition were recorded. The patients were the most severely affected third of the patients acutely admitted to hospital and surviving the acute phase of stroke. In spite of this, 59% of all patients returned to the same kind of living arrangement as before the onset of stroke. The most important factors in a logistic regression model for predicting patients'' return home were a high postural stability score, low age, and absence of perceptual impairment. The prediction model presented here can be quite useful for rehabilitation planning at admission to a geriatric stroke unit and can help to predict discharge disposition and patients'' changes of returning to their own homes.

Involuntary movements including hand tremor are uncommon but well recognized clinical manifestations after stroke. However, head tremor following acute stroke has rarely been described in the literature. I describe 4 patients who developed head tremor after acute stroke. They presented with side-to-side or up-and-down 2- to 4-Hz head tremor that was aggravated on sitting or standing and disappeared on sleeping. Imaging studies demonstrated infarcts (patients 1–3) and a hemorrhage (patient 4) each involving the paramedian pontomesencephalic region that may contain the crossing superior cerebellar peduncle. 摘要:
We report a case of a 72-year-old woman who developed sudden-onset right hemiplegia and global aphasia 8 days after splenectomy performed for Werlhof’s disease. Hematological tests revealed a progressive increase in platelet (PLT) count from the day of the operation to the day of ischemic stroke. Carotid duplex showed a recent left internal carotid occlusion, and transcranial Doppler showed reduction of mean left middle cerebral artery velocities in the acute phase, with subsequent improvement in velocities. This case stresses the importance of rethinking the clinical criteria for prophylactic anti-PLT treatment in elderly patients requiring splenectom
