The pathogenesis of os odontoideum is still a subject of debate. The authors encountered two infants who had traumatic atlantoaxial dislocation and in whom os odontoideum later developed. One of the infants died of tardive cervical myelopathy secondary to cord compression. The authors present their concept that postnatal development of this abnormality results from fracture of the odontoid.

In 85 patients with myelopathy and cervical spondylosis, Queckenstedt's test was used as a guide in deciding upon laminectomy under local anesthesia or anterior surgery under general anesthesia. Laminectomy was carried out in patients with marked block phenomena. The operations were laminectomy (51), anterior surgery (24), and laminectomy plus anterior surgery (10). None of the patients was made worse as a result of the surgical procedure. Improvement in the legs occurred in 73 patients. Block phenomena and arm symptoms were preoperative factors related to improvement in the legs after surgery. Improvement in the arms occurred in 63 of 73 patients who had such symptoms.

Correct dermatome identification is essential to reduce nomenclature and surgical errors, to improve neurologic correlation, and to produce accurate statistics regarding low-back problems. The phylogenetic category of the spine must be known and junctional transitional vertebrae within the column properly identified and localized. This article emphasizes the appearances of, and the different positions of, transitional vertebrae at the thoracolumbar and lumbosacral junctions in the vertebral sequence depending on the phylogenetic category of the patient. It describes three additional characteristics of these anomalous vertebrae. Suggestions are made to eliminate the confusion and mistakes produced by the presence of these frequent variants.

Scar formation after laminectomy was evaluated in dogs treated with hemostatic agents, antiinflammatory drugs, or mechanical barriers. Microfibrillar collagen was found to be superior to gelatin foam. The antiinflammatory drugs delayed healing and frequently resulted in abscess formation, and eventually scar filled the defect. The fat grafts formed a viable mechanical barrier. Gelatin foam sponge was compared with microfibrillar collagen for hemostasis with free fat grafts in a clinical series of 50 patients. The latter technique was clearly superior.

Hair samples were collected from 74 patients with idiopathic adolescent scoliosis and from 25 control children and were analyzed for content of the following minerals: copper, sodium, iron, zinc, potassium, magnesium, cadmium, calcium, and manganese. The hair copper level of the scoliotic children was significantly higher than that of the controls. The scoliosis mean was 6.5µg/dl and the control mean was 3.6µg/dl,P< 0.025. There was no correlation between the amount of hair copper and the severity of the scoliosis. The authors suggest that copper may be a factor in the development of scoliosis since it is part of the lysyl oxidase enzymes that are required for cross-linking of collagen and elastin. Another connection is that postpubertal girls have higher copper levels than boys and also have a greater severity of scoliosis.

Twenty-four cases of lumbosacral hemivertebrae are reviewed. The treatment groups consisted of observation, bracing, posterior spinal fusion with and without Harrington instrumentation, and two-stage hemivertebral excision. Clinical and radiographic spine decompensation was found to be an important parameter in following these patients. Results with observation and bracing were variable and unpredictable. The best surgical correction was obtained with lumbosacral hemivertebral excision.

Cadaveric lumbar intervertebral joints were subjected to physiologic loads to simulate flexion. The resistance to bending was measured first with the joint intact and again after cutting in turn through the supraspinous/interspinous ligaments, the ligamentum flavum, and the capsular ligaments of the apophyseal joints. In this way the part played by each of these structures, and the intervertebral disc, in resisting and limiting flexion was determined. The results show that at the limit of flexion the supraspinous and interspinous ligaments sprain first and that the capsular ligaments and the intervertebral disc offer considerably more resistance than the ligamentum flavum and the supraspinous/interspinous ligaments. The intervertebral joint was found, on average, to be sufficiently stiff to balance about half the bending moment exerted by the trunk in full flexion.

This paper reports on a pilot study of occupational back pain among a sample of 68 men aged 16 to 65 years in the North Bedfordshire Health District, England. In a multidisciplinary approach, measurements of 126 variables for each case were made. It was found that driving high mileages was a significant factor affecting both occurrence and location of pain. Certain personality factors were also significant.

The lumbar shield is a silicone implant designed to prevent postoperative perineural adhesions, indicate a recurrent intervertebral disc rupture, facilitate further surgery at that level, and aid in the postoperative management of the patient. Over three years it has been implanted in 123 experimental patients. There are 83 control patients. Statistical analysis of the results indicates that for the first three months, those patients with the lumbar shield had less postoperative pain. The diagnosis of a recurrent lumbar disc rupture on lateral lumbar spine radiographs is illustrated. The ease of secondary surgery for a recurrent lumbar disc rupture is emphasized. The value of the lumbar shield in postoperative management is discussed, especially in the workmen's compensation/medicolegal groups. One conclusion is that the frequency of multiple secondary surgical procedures on patients with unsatisfactory initial results could be reduced by the use of the lumbar shield.
