AbstractEstimates of the natural distribution of environmental demands on older adults are provided here. Frequencies of occurrence over a one‐year period for 54 life events relevant to older adults are reported for a sample of 1102 men and 1758 women representative of the population aged 55 and older in Kentucky. Estimates of the absolute number of noninstitutionalized Kentucky older adults who would experience selected events in a year are thus made possible. In addition, older adult judgments as to the desirability or undesirability of each event are also reported. These data could be used, with some qualifications, to establish normative weights or desirability classifications for these events for older adults. The following selected classes of events are evaluated in terms of their impact, based on the desirability ratings and previous research: health, death, family conflict, and rewards. Program implications suggested by considering both the prevalence and impact data are: (a) mental health interventions should be closely tied to physical health events; (b) death of spouse would be an event important for prevention programs; (c) other forms of marital disruption would be too infrequent to justify programs; (d) retirement would appear to have too little impact to justify adjustment programs; and (e) the very desirable impact of reward events found here would justify programs promoting these events for older adult

AbstractA conceptual framework for analyzing the role of daily events in the quality of life (QOL) of the elderly is presented. Concepts involving perceptions of control over the causation of daily events are shown to relate to positive and negative aspects of adjustment. Prior research by the authors has shown the utility of assessing the following event sequence: causation of event, response to event, outcome of event activity. The elements in this sequence can be related independently to QOL, and each or all can be differentially impacted by the aging process. The literature on four aspects of aging (retirement, death of a significant other, relocation, and institutionalization) and activity, disengagement, and congruence models are reviewed from an event‐activity approach. Basic information on family forms and the pattern of family support for older adults is discussed. In addition, several examples of interventions with family members of older adults are reviewed. The goal of these activities is to increase the range of options available to older adults and their family caregivers.

AbstractThe literature on natural support systems of rural aged is examined from the perspective of functional specificity. Although family and friends are prominent in the support systems of both rural and urban aged, there are differences in support systems. Further, rural aged have more transportation, housing, and health problems than do urban aged. A number of program recommendations are made that will benefit rural as well as urban aged as future resource availability declines.

AbstractPhyllis AmaralThis critical review is focused on six selected issues relevant to those programs in which older paraprofessionals work with their elderly peers: (a) the overall emphasis on preventive mental health benefits for the helpers via the provision of meaningful roles; (b) the range of roles of older paraprofessionals, including peer counseling, casework and outreach, community organization and gerontological advocacy, and other adjunctive roles in agencies; (c) the wide diversity in recruitment, selection, and training practices; (d) the rationale of indigenousness or helper‐helpce age similarity as uniquely enhancing the effectiveness of older paraprofessionals; (e) the marginal integration of elderly paraprofessional programs into the mental health system; and (f) the paucity of program outcome evaluations. AbstractThere is a growing gap between needed and available mental health services for older adults. This article discusses the training needs, personnel shortages, and lack of effective service programs and proposes some strategies for dealing with the problems. There is a lack of adequate methods for identifying and referring frail elderly persons in need of mental health services. Services that are currently available to the elderly are identified along with factors that may influence their use by older adults and other service personnel. There are shortages of mental health professionals with special training and experience in gerontology and geriatrics, and clinical geropsy‐chologists. An innovative and practical service model that was developed to coordinate and integrate the identification and treatment of frail, community‐dwelling elderly is presented. Various aspects of the program that include outreach and case management approaches, the integration of student volunteer activities with those of special volunteer corps within public housing and specially trained paraprofessionals, telephone reassurance programs, and a system of layered supervision are described. Comparisons are made between this Coordinated Agency Resources for Elderly Services (CARES) model and existing service delivery programs. A three‐tiered approach to training and coordinating the activities of trained mental health personnel: (a) paraprofessionals and volunteers, (b) middle‐level professionals and (c) supervisory professionals within an integrated system is proposed. Specific roles and functions of personnel at each level are described. The summary discusses the potential of the proposed strategies for dealing with the problems created by the shortage of personnel and services.
