Between September 1992 and June 1995, 33 patients with radiolucent urinary calculi were treated in our hospital using Dornier HM-3 and HM-4 lithotriptors. There were 11 caliceal stones, 13 in the renal pelvis, and 9 in the ureter. Stone localization was achieved in only 30 patients using intravenous urography or retrograde urography through a ureteric catheter during the procedure. All patients were treated on an outpatient basis, without anesthesia. During the 1st month following extracorporeal shock wave lithotripsy, the patients received alkalization. The overall stone-free rate at 3 months was 90%. The time of radiation exposure was in the range of the opacified stones. Extracorporeal shock wave lithotripsy using Dornier HM-3 and HM-4 lithotriptors proved to be an effective procedure to treat radiolucent urinary calculi when localization of the stone is possible.

Objective: We have aimed at assessing whether a condensed bleomycin/etoposide/cisplatinum (BEP) regimen would be as effective as the conventional protocol in the treatment of patients with metastatic testicular tumors. Methods: Sixty-four consecutive patients with metastatic germ cell testicular tumors were included into this modified BEP protocol between April 1991 and January 1995. The condensed regimen consisted of a single daily dose of cisplatin (100 mg/m2) and bleomycin (30 mg) plus 3 consecutive days of etoposide (150 mg/m2). Results: Of the 64 patients, 34 (54%) achieved a complete response with chemotherapy alone. An additional 23 (35%) patients in addition to chemotherapy have undergone surgery and became tumor free after resection of the residual masses and additional chemotherapy. The mean hospitalization period of 64 patients who received a total of 289 therapy courses was 65.4 (49–192) h for each course. The creatinine clearances did not differ significantly before and after treatment in neither of the chemotherapy courses. Conclusions: We would thus like to suggest that this condensed BEP protocol is cost-effective and improves patient compliance and quality of life. Furthermore, this is achieved with comparable efficacy to conventional therapy without increasing the toxicity. However, prospective randomized trials with similar results are needed to determine the efficacy of this regimen, and only then this regimen may replace the conventional therap

A cirsoid type of arteriovenous malformation was treated successfully by superselective embolization using a platinum micro coil and absorbable gelatin sponge. During the procedure, as soon as the arteriovenous shunt had disappeared, an arteriopelvic shunt appeared. The pyelogram showed that the high pressure arterial blood flow drained directly into the pelvic cavity. Massive gross hematuria occurred immediately accompanied by a sudden blood pressure drop to 60 mm Hg, necessitating blood transfusion. Fortunately this complication was managed successfully. To our knowledge, there are only a few reported cases that make reference to gross hematuria occurring on embolization. We discuss the possible causes of this complication and the mechanism of occurrence of gross hematuria in renal arteriovenous malformation.

Two rare cases of contralateral adrenal metastases are reported. One with a synchronous presentation where the renal tumor was removed together with a solitary large contralateral adrenal metastasis. The second patient underwent adrenalectomy for a metastatic lesion diagnosed 4 years after nephrectomy performed for renal cell carcinoma of the contralateral kidney. Five years following the adrenalectomy the patient underwent leg amputation because of a solitary bone metastasis.

A 26-year-old man presented bilateral cysts of the vesicles, larger on the right side, associated with agenesis of right kidney and ureter. Surgical treatment consisted of removal of the right cyst and volume reduction of the left one. The embryological aspects are discussed, together with the clinical findings and the treatment options.
