The sonographic manifestations of 20 childhood abdominal biopsy-proved lymphomas were presented. Thirteen cases showed positive renal sonographic findings such as renomegaly, hyperechoic renal cortex, focal hypoechoic mass and hydronephrosis. The incidence of renal involvement was high in lymphoma detected by ultrasound. There was no difference between sonographic findings of Hodgkin and non-Hodgkin lymphoma. The ultrasound findings may be confirmed with gallium scanning. Ultrasound was more accurate in detecting renal lesions in lymphoma than computed tomography.

The effects of prazosin HCl treatment on several variables were determined in 16 patients suffering from micturition disturbances associated with benign prostatic hypertrophy. After administration of the drug in doses of 1–2 mg/day, subjective symptoms improved in 11 patients (68.8%). Urine retention rate, maximum urine flow rate, and average urine flow rate exhibited statistically significant improvement, but voided urine volume did not. Urethral pressures declined significantly in the bladder neck and prostatic urethra but not in the pressure at the external sphincter. To evaluate the accuracy of transrectal ultrasonography in staging and detection of prostatic cancer, a prospective study was initiated. In 466 patients transrectal ultrasonography was performed before transurethral resection, suprapubic adenomectomy, or radical prostatectomy. The findings were compared with the histopathologic results. In 160 of the 466 patients a carcinoma of the prostate was proven by histology. In 21 patients the cancer was an incidental finding after transurethral resection. In 133 of the 160 patients (83.1%) the tumor was detected by ultrasound prior to surgery. The analysis of the results revealed, however, great differences in the accuracy in the different tumor stages. In those patients where the tumor was confined to the prostate, the carcinoma was identified only in 37.5% of the cases. Especially prostatic cancer stage pT1a-b (incidental) was detected only in 3 out of 21 patients. Thus, it can be concluded that transrectal ultrasonography is not more sensitive than the rectal palpation for detection of prostatic cancer stage T1a-b.

Prazosin, a selective α1-adrenoceptor blocker, was compared with Paraprost in a double-blind, multi-institutional study in patients with benign prostatic hyperplasia. The trial encompassed 77 evaluable patients in the prazosin group and 79 in the Paraprost group. Both drugs significantly improved obstructive and irritative symptoms. Residual urine (volume or percentage of total urine) was reduced by both prazosin and Paraprost. Average and maximum flow rates increased significantly in the prazosin group while those were unchanged in the Paraprost group. Side effects occurred in 1 patient treated with prazosin (1.1%) and 3 patients with Paraprost (3.3%). Thus, we confirm the clinical value and minimal side effects of prazosin in the treatment of bladder outflow obstruction resulting from benign prostatic hyperplasia

We have developed a technique of transurethral radiotherapy for prostatic cancer using a remote afterloading system. The radioactive source is a cobalt-60 pellet with 3.7-Ci activity. Four patients with adenocarcinoma of the prostate were treated. In all patients, the local tumor response was rapid and satisfactory as judged by physical examination as well as ultrasonography. Serious complications were not seen. A new technique and preliminary clinical results are reported.

Meshgraft urethroplasty has become one of the standard operative procedures for the treatment of long and complicated urethral strictures. The original method used meshed foreskin for urethral reconstruction. To extend the application of the method to circumcized patients, a split-thickness skin graft was used to construct a neo-urethra. In a first stage, a split thickness skin graft is harvested from the medical part of the thigh and transplanted alongside the opened urethra. After complete healing of this transplant, the neo-urethra is formed in a second stage 12 weeks later. Since 1980 meshgraft urethroplasty using a split-thickness skin graft has been performed in 34 patients. In all patients excellent anatomic and functional results have been achieved. This technique was found to be most useful in exceedingly long or problematic strictures, e.g. in spinal cord-injured patients.

Fifty-eight normotensive patients with benign prostatic hyperplasia and maximum urinary flow rates of less than 15 ml/s were randomly assigned to receive a 12-week course of treatment with prazosin or placebo in a double-blind parallel group trial. Prazosin was administered orally in doses of 0.5 mg and then 1 mg twice daily for 4 days each and 2 mg twice daily for the remainder of the trial. Patients on treatment with prazosin had an increased urinary flow rate as compared to placebo with a significant reduction in maximum voiding detrusor pressure and maximum detrusor pressure at peak urinary flow. Although a significant effect on frequency was not demonstrated, standard parameters of detrusor instability were reduced. Investigators’ double-blind overall assessment of efficacy significantly favoured the prazosin treatment. Twelve patients were excluded from the final analysis, 8 being withdrawn because of adverse effects, 5 on treatment with prazosin and 3 in the placebo group. The value of lymphadenectomy in the treatment of renal cell carcinoma remains controversial. The precise location of nodal metastasis and method of lymphadenectomy had seldom been mentioned. We report 2 patients with right renal cell carcinoma who had regional lymph node involvement without blood-borne metastasis. The involved lymph nodes were demonstrated by CT scan just behind the inferior vena cava at the level of renal hilus. These nodes seemed to be the first sites of nodal involvement from the right renal cell carcinoma. Elevation of the inferior vena cava by dividing its small branches, including a few lumbar veins, was necessary for complete removal of these nodes en bloc with the right kidney.

A 53-year old male with multiple pulmonary metastases from a renal cell carcinoma was treated with high dose recombinant interleukin-2 (rIL-2). rIL-2 was administered intravenously, and the dose was gradually increased to a subsequent daily dosage of 3 × 106 units for a total amount of 470 × 106 units. After 3 months, plain chest film and chest computerized tomography revealed a remarkable reduction in pulmonary metastases. There were no side effects except general fatigue and fever. Immunological studies revealed an increase in lymphocytes and enhancement of natural killer activity. An 8-center, double-blind, placebo-controlled trial of 39 patients with benign prostatic hypertrophy was conducted to assess the effects of prazosin HCl treatment on functional urologic variables after a treatment period of 4 weeks. The randomized groups were comparable for demographic variables and symptoms, except for the mean residual urinary volume, which was significantly higher in the prazosin HCl group. Prazosin HCl elicited statistically significant improvements in the mean prostatic urethral pressure and prostate area (Mann-Whitney U test: p = 0.001 for both variables as compared with the placebo group). Functional bladder capacity also improved significantly in the group receiving the test drug (Mann-Whitney U test: p = 0.05, as compared with the placebo group). Clinical improvements were also observed in the mean maximum urinary flow, decreased nocturia frequency, and residual urinary volume. Patient preference significantly favored the prazosin HCl treatment (Mann-Whitney U test: p = 0.001). Seven patients on prazosin HCl and 5 receiving placebo reported one or more side effects during the trial phase; these were mild to moderate and disappeared or were tolerated. No statistically significant effects on blood pressure or heart rate were observed.
