In order to evaluate quantitative changes in kidney proteins, computer-assisted histophotometry of tissues was performed. Kidney marker enzyme concentrations were considered to be involved in inductive and alterative developments in the proximal tubule. These effects were caused by the administration of aminoglycosides. Indicator enzymes of the proximal tubule such as membrane-bound alkaline phosphatase (AP) and alanine-aminopeptidase (AAP) as well as lysosomal β -glucuronidase (β-Gl) were stained in kidney tissues. Graphic monitoring and digital display of kidney cortex sections were registered by electronic image analysis using the ‘Micro-Videomat’ 2 system. As an experimental design, 160 kidneys of Wistar rats were studied. Their kidneys were analyzed after one, two and three intravenous applications of gentamicin, 25 mg/kg/day, or tobramycin, 25 mg/kg/day, or amikacin, 50 mg/kg/day, on consecutive days. In a fourth test group, the kidneys were examined after the third application and a 5-day recovery period. Concentrations of tubular marker enzymes were significantly increased (2p < 0.01, Wilcoxon test) after two applications of gentamicin: AP 37.6%, AAP 6.3% and β-Gl 11.8%. Contrary to these findings, after two injections of tobramycin or amikacin only slight alterations were documented: tobramycin, AP 14.5%, AAP ––0.1 % and β-Gl 0.4%; amikacin, AP 6.0% and AAP 4.2%. The studies indicate that tobramycin induces less severe nephrotoxic and inductive reaction than gentamicin and partly than amikacin in dosis administered during our experiments. In all cases, enzyme activities studied were nearly normalized after three applications of aminoglycosides and a recovery period of 5 days. Bio-feedback has recently been taken into use in the treatment of diseases of muscular origin. Here it is possible to establish a self-controlling relaxation and straining of muscles. Detrusor-sphincter dyssynergia of non-neurological origin is possibly a functional development disorder well suited for bio-feedback treatment. A bio-feedback circuit and the treatment of a patient with detrusor-sphincter dyssynergia is described in the present paper.

The mechanical activity of isolated strips from different areas of the pyeloureteral system was investigated in 10 pigs: calyx, renal pelvis, pyeloureteral junction and ureter. Additionally, electrical activity was measured in some pyeloureteral preparations using the sucrose-gap technique. Regular spontaneous activity with an average frequency of 9.5/min was recorded in calyceal strips, decreasing to 5.4/min in renal pelvis, 5.7/min in pyeloureteral preparations and to 1.2/min in ureteral preparations. The activity of renal pelvis, pyeloureteral and ureteral preparations was less regular, and bursts of fast activity (near 10/min) could be observed in all these preparations. The membrane potential of pyeloureteral strips showed spontaneous generator oscillations of about 10/min. Variations in the pattern of ureteral peristalsis are due to different coupling ratios of membrane potential oscillations to contractions. Adrenaline (10––5 mol/l) increased the frequency of the oscillations and enhanced their manifestation in the mechanical recordings, whereas tetraethylammonium (5–20 mmol/l) only increased the coupling ratio. The following concept for the generation of ureteral peristalsis in multicalyceal kidneys is developed: several (primary) oscillators exist in the calyces; in the pyeloureteral junction a (secondary) pacemaker exists which has an intrinsic frequency similar to that of the calyceal pacemakers; both processes cooperate in the generation of ureteral perist

Paratesticular embryonal rhabdomyosarcoma was diagnosed in 4 children admitted to our hospital between 1973 and 1978. On the basis of these 4 cases and the reviewed literature the current concept in the management of this tumor is discussed. A combined multidisciplinary approach consisting of radical orchiectomy, retroperitoneal lymph node dissection and chemotherapy is necessary. When retroperitoneal lymph nodes are found to be involved by the tumor radiation therapy of this area should be admitted.

To verify if uremia per se influences the eventual outcome of bacterial challenge, we have set up an experimental model of infection in rats with surgically induced uremia and controls. Our preliminary results indicate that both uremic and control rats die at the same rate when challenged intraperitoneally with LD50 of a Pseudomonas strain. These observations would suggest that uremia per se is not a major determinant of the increased incidence of infectious complications as has been noted in this condition.

The clinical experience and results of NPT recording on three different groups of patients (a group of normals, a group with high suspicion of psychogenic impotence and a group of organic impotence with a well-known etiology) are reported here. A new classification of erectile capacity which is not only based on change in circumference of the penis, but also on duration of complete erection and rigidity of the penis is proposed. The phalloplethysmography, which is based on the REM-related tumescence is a reliable method for discrimination between organic and psychogenic impotence. A rigidity test, however, is essentially for the detection of insufficient rigidity in spite of normal penile expansion during NPT recording.

In the diagnosis of urethral stenosis before prostatectomy, urethrography and urethroscopy were independently compared in a prospective series of 100 cases. Total agreement was found in 94%. In 6% urethrography demonstrated minor abnormalities not found at urethroscopy. Unexpected lesions were found in 16 %.

Prostatectomy is an operation a lot of men have to undergo. Because this is an intervention which involves the intimate sphere of a man, we tried to investigate the postoperative sexual problems of 200 men prostatectomised without complications. As a result, we would like to point out that exact preoperative information by a doctor is very important. With these precautions, no impairment of sexual functions is to be expected.

Recurrent urinary infections can be connected with unilaterally shrunken kidneys and they frequently can be removed by nephroureterectomy with exception of reflux-induced shrunken kidneys. Insufficient kidneys, caused by reflux and developed in early childhood, are not the source of infection, in general. Hypertension, induced by the vascular or parenchymal process of unilaterally insufficient kidneys, frequently can be removed by nephrectomy. But the renin quotient is not a reliable prognostic value. Therefore we will bring into question the investigations of renin concentration in both renal veins and peripheral blood circulation.

5 children with yolk-sac tumor of the testis (= infantile embryonal carcinoma) were admitted to our hospital between 1973 and 1981. Evaluation of serum alpha-fetoprotein concentrations and ultrasonography of the retroperitoneal area proved to be extremely valuable to predict the tumor stage in these children. Therapy consisted of high inguinal orchiec-tomy, retroperitoneal lymphadenectomy and cytostatic chemotherapy in 3 children using the T2-protocol. All children are now between 1 ½ and 8 years free of recurrence and metastases. Therapy of this tumor ought to be differentiated according to the individual tumor stage.
