With a new enzymatic method, the dietary influence of oxalate, glycine, protein, and ascorbic acid on serum and urinary oxalate has been examined. Healthy and oxalate stone-forming subjects were compared. Two doses of sodium oxalate (130 and 400 mg daily) were administered. The high dose induced significant hyperoxaluria. No changes of serum oxalate were seen. Neither glycine (4.5 g daily) nor protein (50 g daily, 50% animal protein) had any effect on serum or urinary oxalate. Urinary oxalate excretion did not increase upon ingestion of large amounts of ascorbic acid (1–6 g daily), but serum oxalate levels were significantly elevated. Carcinoma in situ of the renal pelvis in a patient with an ileal conduit is reported. By fiberendoscopy, using gastrointestinal instruments, both ureters were catheterized. Cytological investigation of urine from each kidney revealed that malignant cells were exfoliated from the left side exclusively. The correct diagnosis of a carcinoma in situ of the left renal pelvis was confirmed by a histological examination of the operative specimen.

A urodynamic analysis was performed on 1,300 patients to precise typical psychogenic voiding patterns. Only 2.7% of this selected group showed voiding alterations derived from a psychosis or neurosis. The psychosomatic voiding alterations were to some degree similar to the alterations which were provoked by exogenous or iatrogenic influences. The voiding pattern of young children differs from that of adults when comparing psychosomatic symptoms such as prostatitis or enuresis. Urine retention and megalocystis were characteristic findings of psychotic patients whereas a polyphasic flow pattern, a delay of micturition and increased sacral reflex activity were typical findings of psychosomatic disorders. The denervation supersensitivity test is a reliable test to differentiate neurogenic from psychogenic voiding patterns.

Patients with reduced renal mass (i.e., nephrectomized) and slightly decreased renal clearance (70–80%) have been treated by 4 g DNA to study uricosuria. Prompt or prolonged renal excretion of uric acid and creatinine have been observed with indirect effects on parathyroid secretion. Urinary cAMP, however, failed to correlate to the change of parathyroid function. Reversely, PTH injection significantly increases uricosuria suggesting interrelated secretion of uric acid and cAMP of the proximal tubules. The early stages of urethral stricture formation have been examined by light and electron microscopy after injury of the anterior urethra in the guinea pig. It was found that initially an ulcerated surface formed which was subsequently covered by simple then stratified squamous epithelium. The tissue of the stricture contained elongated myofibroblasts and clumps of multinucleated giant cells. It is suggested that the myofibroblasts are responsible for the formation of the stricture and that the giant cells may be involved in continued collagen synthesis.

It takes 24 months to induce preinvasive carcinomas in the urinary bladder of dogs as reported in the literature. The transplantation of chemically induced carcinomas has never been successful. Therefore, it was looked for methods which enable a shortening of the latency period and to transplant such tumors. 12 of 22 female beagle whelps were injected transurethrally with fast polymerizing plastics and then they have been treated subcutaneously with ortho-aminodiphenyl, formylaminonitrofurylthiazole or with the two carcinogens together. The dogs have been controlled by endoscopy, cytology and histology every 4 weeks. The first invasive urothelial carcinomas were observed 13 months after starting the treatment. Totally 7 carcinomas of the bladder and 3 in the pelvis of the kidney were found. It was possible to cultivate tumor material in tissue culture, gained by biopsy transurethrally. The transplantation of tissue culture material into nude mice was successful in 19 of 23 animals. In 2 of 10 beagles tumors started to grow transiently after transplantation of tissue culture cells. In the bacterial mutagenicity test (Ames) and in animals ortho-aminodiphenyl was neither mutagenic nor carcinogenic. However, it was cocarcinogenic if used together with formylaminonitrofurylthiazole.

The light and electron microscopic structure of biopsy samples of trabeculated urinary bladder from patients with proven outflow obstruction due to prostatic hypertrophy has been compared with the morphology of control bladder specimens. In the latter the detrusor muscle bundles were composed of smooth muscle cells closely packed together with very little intervening connective tissue. In contrast, irrespective of age, detrusor muscle from trabeculated bladders contained many muscle bundles in which the constituent cells were of relatively small diameter and were widely separated from each other by dense masses of connective tissue. No morphological evidence for smooth muscle hypertrophy or hyperplasia was obtained in the present study. In the electron microscope the connective tissue between the smooth muscle cells of trabeculated bladders was seen to contain, in addition to collagen fibrils, an extensive meshwork of electron-dense microfibrils apparently in continuity with the basal laminae of the smooth muscle cells. Regions of close approach between smooth muscle cells were seemingly unaffected by trabeculation as was the distribution and fine structure of autonomic nerve terminals. These observations are intended to form a baseline for comparison with the results of future morphological studies of trabeculation arising in response to different aetiological factors.

Urinary incontinence in 80 men was classified by combined radiological and urodynamic techniques. 28 cases of pure stress incontinence were evaluated for grading of sphincteric loss. Clinical and sphincterometric data were correlated. Functional urethral length, urethral closure pressure at rest and with voluntary sphincter activation, and pressure transmission under stress were used to estimate the remaining sphincteric functions. Theoretical guidelines are drawn for the use of operative sphincter repair based on the grade of stress incontinence.

Patients with benign hyperplasia of the prostate (BHP) were studied in relation to aberrant serum c-peptide, testosterone, estrone, cortisol and morning fasting glucose, to analyze the syntropy of diabetes and BHP. The results were compared to patients with bladder cancer without BHP and, after correction of the patients’ weights and ages, the control failed to prove any significance. This is consistent with the incidence rate of maturity onset diabetes in prostate adenomas (n = 527, 7.3%) and of a control group (8.5%) of patients, 60–70 years of age. Zinc, another parameter associated with insulin function, is frequently observed to change in BHP and is significantly increased in prostatic tissue which possibly acts as a zinc pool. In this study, however, no effect of prostatectomy on serum zinc was demonstrable even in a long-term control of 9 months, suggesting a sensitive and rapid regulation of this metal independent from the prost
