In 7 patients undergoing general anesthesia and transurethral resection of the prostate, the effective renal plasma flow (ERPF) was recorded before, during and after anesthesia using a new isotope feedback technique. The isotope we used was I131 iodopyracet. In comparison to the average preanesthetic I131 iodopyracet clearance (319 ml/min), the clearance measurements during general anesthesia in 7 patients revealed a fall in ERPF of 32%. In 5 of these patients receiving pentothal-N2O-oxygen anesthesia there was a decrease of renal plasma flow of 30%. Epidural anesthesia was attempted in the remaining two patients but proved to be insufficient. One of these patients received nitrous oxide-oxygen anesthesia which caused a fall in ERPF of 31%. To the second patient halothane-N2O-oxygen anesthesia was given, which was followed by a decrease in renal plasma flow of 44%. A more detailed study revealed a fall of 29% in iodopyracet clearance during general anesthesia before operation and a fall of 35% during transurethral prostatectomy. After general anesthesia we found in all patients a marked increase of renal plasma flow.

A review of the neurosurgical procedures which may be helpful in the treatment of bladder dysfunction resulting principally from lesions of the nervous system is presented. A summary of the indications, contraindications, rationale and operative technique for each procedure is described. Neurogenic bladder dysfunction is considered in terms of the balance between the forces of expulsion and those of retention and the underlying anatomical and physiological relationships are reviewed.

Hundred and fifty spinal cord injury patients were interviewed concerning erections, coitus, ejaculation and orgasm. 123 (82%) were able to attain erections. 115 patients had upper motor neuron lesions (reflex activity present in the sacral segments). Seventy-five of the 115 patients had complete lesions; 9% had psychic erections, 95% had spontaneous erections, 92% had erections from penile stimulation. 44% of the patients made no attempt at coitus. Forty-two patients attempted coitus of which 24 (57%) were successful. One percent could ejaculate and have an orgasm. 40 of the 115 patients had incomplete lesions. 48% had psychic erections, 93% had spontaneous erections and 93% had reflex erections. 27% of the patients made no attempt at coitus. 29 patients attempted coitus of which 25 (86%) were successful. 25% could ejaculate and have an orgasm. Thirty-five patients had lower motor neuron lesions (reflex activity absent in the sacral segments). 33 of the 35 patients had complete lesions; 24% had psychic erections, 12% had spontaneous erections. 36% of the patients made no attempt at coitus. 21 patients attempted coitus of which 7 (33%) were successful. 15% could ejaculate and have an orgasm. Two of the 35 patients had incomplete lesions and one of the two had psychic erections. One patient made no attempt at coitus. One patient who attempted coitus was successful and could ejaculate and have an orgasm. All of the patients with complete or incomplete lower motor neuron lesions experienced psychic erections only then when the lesion was below T-12. The activity or hyperactivity of the isolated sacral segment of patients with complete upper motor neuron lesions appear to have an inhibitory effect on ejaculation. Pinprick perception in the sacral segments of patients with incomplete upper motor neuron lesions seems to be prognostically more significant for erection and ejaculation than the preservation of light touch perception or retained volitional motor function of the pelvic floor muscles. Various observations are reported, such as the relationship of spasticity and sexual function, autonomic dysreflexia during sexual activity, premature aging in regard to sexual function, and some interesting psychosexual attitudes reported by the patients. This study has essentially confirmed the findings noted in the original study from this Center.

On the basis of 11 cases of intraepithelial carcinoma of the bladder the bibliography on this subject is revised and the clinical, endoscopic and histopathological features are discussed. Intraepithelial carcinoma may appear clinically as a chronic stubborn cystitis without true tumour or may be preceding, subsequent or concomitant to a bladder tumour. From the endoscopic point of view it appears as red patches in the mucosa and small yellowish granulations that are in fact neoplastic v.Brunn’s nests. By microscopical observation, the epithelial cells show neoplastic features with superficial extension but without submucosa or muscular infiltration. We report our experience with 6 cases of severe bleeding of the urinary bladder, due to the cytostaticum cyclophosphamide. In 3 cases a thrombocytopenia was proved. One patient developed a shrunken bladder with bilateral reflux. If the bleeding does not stop after discontinuation of cyclophosphamide, introduction of a catheter, removal of clots and administration of whole fresh blood, then a simple urinary derivation operation is advised.
