A new method of ureteropyelostomy with flaps with an everted suture is described. From 1969 to 1973 this surgical technique was performed on 29 patients with hydronephrosis. In all cases the results were very satisfactory.

Caudal lymphography was performed in 80 patients suffering from histologically determined carcinoma of the prostate of different degree and stages of malignancy. Metastases of the lymph nodes occur earlier and more frequently than visceral or skeletal metastases. 39% of our patients showed metastases in the lymph nodes and only 20% metastases in the skeleton. The more dedifferentiation of the tumors increases, the more frequently occur metastases in the lymph nodes. Before lymphography only 20% of the cases belonged to stage D, after it, however, the number of the patients in this unfavourable stage had doubled. Due to caudal lymphography an exact classification of stages in the carcinoma of the prostate can be achieved. It helps differentiating the curative from the palliative stages, thus influencing the therapeutic plan. Moreover, it is useful for establishing prognoses and controls of the course.

Report on a comparative clinical follow-up study to evaluate late results (up to 4 years) of 48 poor surgical risk patients treated by cryosurgery of the prostate (CSP) and 61 patients randomly selected from 306 patients which were treated by transurethral resection of the prostate (TURP). Only extremely elderly and risky patients should be considered candidates for CSP as this offers only limited advantages over sole palliative procedures of urinary diversion or relief of urinary obstruction. If the patients do not belong to the poor or very poor surgical risk category, patients usually do far better after TURP than after cryosurgical intervention.

A case of sarcoma botryoides in a 3-year-old boy with a more than 7 year survival so far after total cystectomy and bilateral ureterosigmoidostomy lead to review the literature of this type of tumor. The indication for the surgical procedure is based on the clinical findings only and does not require histological confirmation. Primary total cystectomy is considered the treatment of choice. With this management 21 out of a total of 152 cases of sarcoma botryoides reported in the literature are alive after total cystectomy and supravesical urinary diversion. Local excision of the tumor as well as radio- and chemotherapy are regarded as unsuccessful in the treatment of sarcoma botryoides.

Three cases of lymphocele were encountered in a series of 347 renal transplantations (0.9 %). In all cases, allograft function was impaired, but surgical intervention restored normal renal function. Careful ligation of all lymphatic vessels during transplantation is of importance for prevention. Simple drainage with a closed system is the treatment of choice in uncomplicated cases.

In cases with untreated bladder carcinoma, mean levels of SLDH and SLDH-V were significantly higher than that of the control, but frequency of abnormally elevated cases were observed in only 19 and 41 %, respectively. Owing to hyperglycemic condition, both enzyme levels in untreated bladder carcinoma cases were significantly lower than in pretreatment levels. In cases with abnormally high pretreatment level, frequency of cases which decreased more than 20 % were 57 % for SLDH and 73 % for SLDH-V, respectively.

Clinical analysis on 285 documented cases of renovascular hypertension reported in the Japanese medical literatures revealed Takayasu’s syndrome occupied 30%, followed by fibromuscular hyperplasia 18% and atherosclerosis 13%. Some of the subjects were compared with that of the Cooperative Study of Renovascular Hypertension in USA. Influenced by Takayasu’s syndrome, renovascular hypertension in Japan has younger age distribution and more female preponderance than that of the Euro-American countries. Stenosis of the distal urethra appeared to be the cause of recurrent urinary infection, diffuse troubles of the abdomen and enuresis in 168 of 272 girls (61.6%). Diagnosis was made by calibration with bougies à boule. Alterations of the urethra on voiding cystourethrogram are no definite criteria for stenosis. Treatment consisted of internal urethrotomy with the Otis urethrotome in 120 cases. 1–2 years after treatment 76 patients (63.2%) were cured, fair results were noted in 21 cases (17.5%). Recurrent urinary infection was removed in 70.7%, vesicoureteral reflux disappeared in 44.8% by internal urethrotomy alone. Succinimide, phenyl lactate, n-heptanoate, oxamic hydrazide, 2-oxoglutarate and ethinyl oestradiol all failed to reduce the urinary excretion of oxalic acid in male Wistar and Sprague-Dawley rats. The problem of controlling oxalate excretion in patients with primary hyperoxaluria or idiopathic calcium oxalate stones is discussed and it is suggested that dietary measures, combined with orthophosphate therapy, may offer a more fruitful approach to treatment than attempts to reduce the endogenous synthesis of oxalate.
