Thyrocalcitonin is a recently discovered thyroid polypeptide which inhibits the efflux of skeletal calcium and thereby can rapidly lower blood calcium. The discovery and known properties of thyrocalcitonin are reviewed. It is postulated that its physiological function is to prevent even transient hypercalciuria and the threat of calculus formation. It can bring about rapid control of incipient hypercalcemia, avoiding an increased glomerular filtration of calcium, and at the same time permit the continued effect of long-acting parathyroid hormone on the renal tubule which achieves the removal of nearly 99% of the filtered calcium from the urine. The therapeutic use of thyrocalcitonin in patients threatened by hypercalcemia or hypercalciuria is anticipated.

Planning the schedule for the blood pressure control after the removal of pheochromocytoma with the NA drip method or with the blood overtransfusion method is described. Advantages and disadvantages of these methods are discussed. It is presumed that the following three points must be important on the postoperative treatment of pheochromocytoma; 1. A maintenance of the cardiac output. 2. A reasonable tension of the peripheral artery. 3. An improvement of the blood volume reduction. From the results presented in this paper, the conclusion could be made that the each method is not always complete when either one method is used alone. Bei je 225 Fallen von Elektroresektion und suprapubischer Operation der Prostata wurde der postoperative Harnstoffanstieg bestimmt. Dabei kam es in 59,5% der Fälle zu einem Anstieg, in 40,5% zu einem Abfall bis zum 3. oder 4. postoperativen Tag. Einen Anstieg von mehr als 40 mg% Harnstoff wiesen 12 der Elektroresezierten und 24 der nach Harris-Hryntschak Operierten auf. In alien Fallen wurden nach etwa 14 Tagen spätestens die praeoperativen Werte wieder erreicht oder unterschritten. Die Ursache dieses Anstiegs wird vor allem in schockähnlichen Kreislaufreaktionen gesehen, die je nach der Operationsmethode verschiedene Ursachen haben können. Die hierdurch hervorgerufene sogenannte «funktionelle Nephropathie» ist reversibel. Der Schluβ scheint berechtigt, auf Grund des geringen Harnstoffanstieges auch Prostatakranke mit Retention (bis etwa 200 mg% Harnstoff und 4 mg% Kreatinin) der Sofortoperation zuzuführen, um die durch eine praeoperative Dauerkatheterbehandlung unvermeidbare Harninfektion zu

Physiological and diagnostic aspects of stress incontinence is discussed. The importance of adequate stretching and fixation of the urethra in operative correction is stressed. The operative method is described with particular attention paid to the suture technic employed. The results of 31 corrections in 30 patients are reported with complete cure in 80% and improvement in the rest of cases. Retropubic suspension is recommended as the method to be used primarily in the correction of stress incontinence.

Bericht über 10 Patienten, bei denen bei der Zystektomie beide Harnleiter in den praevesikalen Raum eingeleitet wurden, den eine ballonartige Einlage offen hielt. Urn den Ballon bildete sich ein neues Harnreservoir, eine Pseudoblase, mit einem Fassungsvermögen von 80–100 ml. Alle Patienten haben den Eingriff gut überstanden. Zwei von ihnen leben bereits über ein Jahr ohne sonderliche Beschwerden. Sie können den Harn gut halten. Die Miktion erfolgt, ohne daβ die Bauchwand komprimiert oder die Bauchpresse starker eingesetzt werden muss. Der Urin ist zwar trübe (Pyurie), jedoch schleimfrei. Das Hauptproblem bei dieser Operation liegt noch darin, eine Stenosierung der Harnleiterenden sicher zu verhindern. Mach Ansicht der Autoren ist diese Art der Zystektomie bei solchen Patienten mit Harnblasenkarzinom angezeigt, bei denen die Zystektomie dringlich erscheint (z. B. bei unstillbarer Tumorblutung) der Zustand der Patienten aber eine grosse eingreifende Operation verbietet, wie sie der Blasenersatz durch eine Darmschlinge d

A report is presented on a patient in whom the intravenous urogram, obtained during an acute attack of renal colic due to an ureteral stone, disclosed peripelvic extravasation. Similar reports in the literature are relatively rare. It might be that such extravasation in acute obstruction can be more frequently observed when intravenous urography is done during the acute phase of the obstruction. This is suggested also by the observation of a slight degree of pericalyceal extravasation during compression in intravenous urography. This extravasation arises from small ruptures in the fornices and from this point continues via several possible pathways, viz.: the pyelolymphatic, pyelovenous, pyelointerstitial as well as the pericalyceal and peripelvic.

Im Tierexperiment wurde in zwei Versuchsserien geprüft, in welcher Form und Ausdehnung morphologische Veränderungen in der Niere, der Wand des Nierenbeckens und des Harnleiters nach längerer Liegedauer von Uretersonden auftreten. Bei der ersten Versuchsanordnung wurdenNíerenbeckenundHarnleitertransvesical aber nach auβen drainiert. Bei der zweiten Versuchsanordnung blieb die Drainage in der Harnblase liegen. In beiden Versuchsserien hat die funktionelle Harnstauung und der Reflux mit und ohne antibiotische Behandlung eine mehr oder weniger starke interstitielle Nephritis und Ureteritis erzeugt. Die bei der Spülbehandlung nach der von Timmermann angegebenen Methode auftretenden Komplikationen wie Fieberschübe, Pyurie, Haematurie, Blutbild- und Senkungsveränderungen werden damit erklärt.
