In the course of our histological studies on the testes of idiopathically infertile men, we found an increased number of mast cells in the limiting membrane of the seminiferous tubule. In 16 patients with idiopathic infertility, quantification of mast cells within the limiting membrane of the seminiferous tubule was performed. There apparently was a tendency to increased numbers of mast cells in the seminiferous tubule wall with decreased sperm count and an average Johnsen score for each tubule. These findings suggest that quantification of mast cells in the seminiferous tubule wall may be a useful marker in the histopathological evaluation of testis biopsies in patients with idiopathic infertility.

Seminal plasma from 22 men attending an infertility clinic was subjected to preparative ultracentrifu-gation for 2 h at 105,000 g. The pelleted material as well as the supernatant thus obtained were investigated with regard to prostasome membrane-linked enzyme activities in relation to other semen parameters. The mean activity of Zn2+-dependent adenosine triphosphatase in the sedimented prostasome fraction was 1.45 ± 1.02 μmol (range 0.29–4.79) orthophosphate released per milligram protein and 20 min, while the corresponding figures for the supernatant were 0.56 ± 0.30 (range 0.12–1.29). Hence, 72% of the specific activity was sedimented, and 28% remained in the supernatant. The same pattern was recognized with regard to the other two enzymes investigated, although they displayed individual characteristics with regard to distribution after ultracentrifugation. The pelleted prostasome-linked mean aminopeptidase activity was 0.39 U/mg protein (81.9%), with only 0.087 U (18.1%) remaining in the supernatant. The corresponding figures for γ-glutamyltransferase were 7.89 (60.4%) and 5.17 (39.6%) μkat/g protein, respectively. The different enzyme activities in the prostasome fraction and supernatant, respectively, were interrelated to each other and correlated significantly with r values between 0.73 and 0.93 (p < 0.001). It was concluded that a minor fraction of prostasomes remained in the supernatant after ultracentrifugation. A relationship existed between prostasomes and semen volume revealing a rather consistent pattern in that small volumes favoured the presence of comparatively more prostasomes in the supernatant and less prostasomes in the pelleted fraction than large volumes. In addition, the sperm concentration seemed to be another determinant of the distribution of prostasomes in seminal plasma on subsequent ultracentrifugation.

Our treatment of acute renal cellular rejection in patients on ciclosporin (cyclosporin A) involves the almost complete elimination of T cells (according to individual T cell monitoring in peripheral blood) through antithymocyte globulin (ATG) therapy. We present here the results of treatment of 44 histologically or cytologically proven acute rejection episodes in 33 of 62 consecutive renal transplantation patients. ATG therapy resulted in a 95.5% success rate when duration and dosage were individually adjusted according to T cell elimination in peripheral blood. Only two grafts were lost, both because of vascular occlusions. In neither case could it be clarified whether the loss was due to primary vascular lesions or the result of severe rejection or side effects of ATG. Our data show that the elimination of primed T cells with ATG permits the use of ciclosporin even in the presence of an established immune response.

In order to assess the normal intrinsic innervation pattern, an enzyme histochemical acetylcholinesterase and two immunohistochemical (S100 protein and neurofilaments) stainings were done on 38 normal urinary bladder specimens. The nerve density was calculated as a mean number of nerve fibers counted per high power field. S100 staining after adequate fixation proved to be similar to the acetylcholinesterase technique, avoids freezing manipulation, is easier to read and permits normal conventional histological examination. Neuropathology of bladder biopsies is an easily available diagnostic method in current neurourological practice.

The functional activity of the sphincter muscle of the urethra is known to be controlled largely by the hypogastric and pudendal nerves. It remains unknown, however, whether innervation of the muscle by these peripheral nerves is ipsi- or bilateral. In an attempt to answer this question urethral closure pressure was determined simultaneously in the anterior, posterior, right and left portions of urethral wall in dogs. The pressure measurements were stereographed with the aid of a computer (stereo-UPP) and by this means the effect of unilateral section or electrical stimulation of hypogastric and pudendal nerves on the intraurethral pressure profile was analyzed. Unilateral section or electrical stimulation of the hypogastric nerve, distal to its division, produced a fall and a rise primarily in proximal intraurethral pressure, respectively, in all four directions. There was no significant difference in this response between the involved and uninvolved sides. Unilateral section of the pudendal nerve resulted in a fall primarily in distal intraurethral pressure in all four directions. No significant difference was present between the injured and noninjured sides. In contrast, electrical stimulation of the pudendal nerve distal to the point of its division caused a rise in intraurethral pressure in all four directions, with a significantly greater pressor response on the stimulated than on the nonstimulated side. These observations suggest decussating motor innervation of the urethra by the hypogastric nerves and also the possibility of the distal urethra being ipsilaterally innervated by the pudendal nerve.

Cystometry at different infusion rates was performed in 7 rats in vivo. The bladders were furthermore allowed to relax from different transmural pressures. When distension of the bladder was moderate, relative relaxation was small and almost independent of transmural pressure and infusion rate. When distended to large volumes, however, there was a marked increase in bladder wall relaxation and the relaxation increased with increasing infusion rates. These findings indicate the importance of standardization of the cystometric procedure.

Twenty-five patients with transitional cell carcinoma of the bladder have been treated with combined therapy consisting of microwave regional coagulation and intracavitary irradiation. A remote-controlled after-loading system was utilized for the radiation therapy. The follow-up period ranged from 6 to 19 months with an average of 11.4 months. Tumor stages were Tis (n = 2), Ta or T1 (n = 17), T2 (n = 2), T3 (n = 3) and T4 (n = 1), and grades were G1 (n = 10), G2 (n = 11) and G3 (n = 4). In 23 patients (92%), there was no endoscopic or histologic evidence of tumor after the initial treatment. Heterotopic recurrences were found after 2 or 3 months in 3 patients who received microwave regional coagulation or intracavitary regional irradiation. Additional intracavitary whole bladder mucosal irradiation was performed for 10 patients with multiple tumors and frequent recurrent tumors. Nine patients had no recurrence (average follow-up 11 months). Our preliminary findings indicate that combination therapy of microwave coagulation and intracavitary irradiation is a useful treatment for bladder cancer.

Acute bacterial prostatitis caused by common urinary tract pathogens is an infrequent disease, and diagnostic difficulties are rarely encountered. On the other hand, chronic prostatitis is a common disease requiring rather elaborate diagnostic procedures. We applied the localization protocol of the four-specimen technique and combined quantitative determinations of microorganisms and quantitative cytologic analysis plus, in chlamydial infections, serologic investigations. Our studies provide good evidence that Ureaplasma urealyticum and Chlamydia trachomatis must be considered etiologic agents in many cases of chronic bacterial prostatitis. These unconventional microorganisms are assumed to infect the prostate by way of intracanalicular ascension from the urethra.

This report is concerned with the experience gained with two 2,8-dihydroxyadenine (2,8-DHA) stone patients. When adenine phosphoribosyltransferase (APRT) deficiency is suspected, the risk of stone formation can be detected at an early stage from the crystalline urinary sediment. Infrared spectroscopic analysis of the crystals or of a urinary stone, if present, will confirm the diagnosis. Determination of the APRT activity will facilitate quantification of the enzyme deficiency and elucidation of the hereditary history. 2,8-DHA excretion in the 24-hour urine and its circadian rhythm were determined at 3-hour intervals using a new method of high performance liquid chromatography determination. This method also provides a means of monitoring the effectiveness of allopurinol therapy.

A new device has been developed for microwave coagulation of urinary bladder tumors. Twenty-one patients with urinary bladder tumors were treated by irradiation with microwave energy of 2,450 MHz. Results were obtained as follows: (1) microwave coagulation was performed in 21 patients with transitional cell carcinoma of the urinary bladder. Excluding 4 patients who subsequently received radical cystectomy, 17 patients showed a complete response, although 2 patients subsequently developed recurrences in different parts of the bladder within the following several months. Histological examination of the excised specimen revealed complete eradication of the tumor in 2 patients. In the remaining 2 patients with high-stage tumor (T4), viable tumor cells were noted in the urethra or vaginal wall. (2) Although neither technical difficulties nor severe complications were encountered, transient urinary frequency and calcification of the bladder wall were noted. The results of this study indicate that microwave coagulation may be used in the treatment of both superficial and invasive tumors.
