The authors have succeeded in passing the fiberscope up to the kidney by the transurethral procedure and taking photographs and movies of renal calyx, pelvis and ureter for the first time in the world. Our long cultivated dream on pyeloureteroscope has come true. When these improvements, which the authors just mentioned, have been achieved and an ideal pyeloureteroscope has been accomplished, tiny lesions of kidney, which have not been diagnosed by the current clinical methods, would be discovered in the early stage of the disease. In the future, this fiberscope would be so equipped with accessory apparatuses that it could be used for the minor treatment or biopsy of the lesions in the upper urinary tract. Therefore, the authors believe the fiberscope would contribute tremendously to the progress of medicine and happiness of mankind.

Measurements of urinary lactate dehydrogenase (LDH) activity in 20 patients with clinically significant unilateral nephroptosis demonstrated a posture-dependent change. While these patients were supine – over night – the urinary LDH activity was normal. On assuming an erect posture, the LDH activity rose to abnormal level. Such ‘swinging’ changes in urinary LDH activity could not be detected in normal people and in patients having undergone nephropexy 1 year following the intervention. Renal ischemic changes at the cellular level were studied in difficult renal stone surgery cases requiring renal pedicle occlusion for various time intervals. By correlating these findings with the clinical course and subsequent return of kidney function, an attempt has been made to determine the safe clamping time. The characteristic findings of renal cell carcinoma noted as specific cell membrane changes verified those seen by other investigators. In this experience the proximal renal tubule appeared to be the primary cell origin of the neoplasm. Papillary transitional cell carcinoma of the renal pelvis was found to be analogous to that seen in bladder cancer. The scarsity of tight cell junctions found in these cases leads to the speculation that the cancer cells easily detach themselves and seed in the ureter and bladder. The epithelium of the polycystic kidney undergoes extensive changes as one would expect in terms of markedly reduced function. The ultrastructural details may have been modified by extrinsic pressure influences and therefore cannot be easily explained. A fruitful field for further study has been revealed.

Large longitudinal nephrotomy (so-called section-incision) is to regarded as an alternative to intrasinusal pyelotomy [Suracco and GIL-VERNET, 1965] in markedly inflamed kidneys with staghorn calculi and in cases of recurrent staghorn calculosis. The section-incision is well tolerated by the kidney and, in our practice, in contrast to staghorn calculous removal via the pyelon, results in less permanent disturbances of outflow at the ureteral junction and calyceal neck. To produce intermittent ischemia the Rumel tourniquet is employed at section-incision. Comparative experience is reported in 15 cases of staghorn calculous removal via the pyelon, in part extended intrasinually, and 17 cases done by the section-incision method.

Angora goats at 110 ± 5 days of gestation were operated on and indwelling catheters were placed in the fetal hearts, amniotic sac and fetal urinary bladder. Venous blood samples from mothers and fetuses were analyzed for Na, K, Cl, and creatinine phosphate. Also, fetal urine and amniotic fluid samples were analyzed for the same parameters. Fetal and maternal electrolytes were found not to be in isotonicity. Fetal urine and amniotic fluid showed significant differences in potassium and creatinine phosphate. The use of amniocentesis in estimating fetal maturity should be carefully evaluated

The clinical staging of bladder carcinoma by the TNM system was studied in 211 patients. Due to difficulties in clinical staging based upon the findings of cystoscopy, bimanual palpation with the patient under anesthesia and biopsy alone, the diagnostic accuracy of roentgenographic procedure was evaluated. The value of bladder angiography, cystography and peripneumocystography combined with tomography in the assessment of tumor size and the extent of perivesical infiltration, as well as the value of lymphography in the demonstration of lymph node metastases was studied in a group of 51 patients. The comparison of the staging based upon clinical criteria alone, with the combined clinico-roentgenological classification showed that the clinical classification results in understaging of patients having an infiltrating bladder carcinoma. The importance of an accurate pretreatment staging of bladder tumors for the appropriate selection of treatment modality was stressed, in order to increase the accuracy of staging of bladder carcinoma by the TNM system, a combined clinico-roentgenological classification was proposed.

As examples of motivation underlying their actions, two young patients are described who were operated because of foreign bodies in urethra and bladder associated with masturbation. Numerous factors of social environment and individual development evince themselves as significant for such foreign body manipulation in which, above all, sexual enlightment must be given a special weight in prophylaxis. In prognosis somatic complications and disturbances of psychosexual development must be taken into account. Apart from urologic operative therapy there is the necessity for juvenile psychiatric follo-up treatment.

The authors collected their material from 12 urological departments. 126 patients from 133 were treated in some form or other. It seems of the best therapy the amputation of the penis completing with block-dissection and irradiation. Results of the treatment show that beside the immediate therapy of the patients must be lay greater stress upon the post-operative care and control.
